On December 7th, 2022 at 10:38AM Licensing Program Analyst (LPA) Rosie Breault, made an unannounced inspection to conclude the complaint listed above. LPA met with Director Beth Hassenplug and explained the nature and purpose of the inspection. Director provided LPA a tour of the facility both inside and out. At the time of the inspection there were thirty-nine (39) children present and twelve (12) teachers.
The Department obtained allegations that a staff member improperly fed a child, and facility does not provide a safe environment for children.
Interviews with parents could not corroborate the allegations and all interviewed indicated they are satisfied with the care their child is receiving and would recommend the facility to other families. Interviews with staff could not corroborate the allegations.
Based on LPA’s observation, file review, and interviews conducted, the preponderance of evidence has not been met, therefore the above allegations are found to be UNSUBSTANTIATED.
Exit interview was conducted with site supervisor and director.
Notice of Site Visit (LIC 9213) posted during LPA inspection, and must remain posted for 30 consecutive days. Failure to maintain posting as required will result in a civil penalty of $100.00.
Appeal Rights were provided.

On December 7th, 2022 Licensing Program Analyst (LPA) Rosie Breault, made an unannounced inspection to conclude the complaint listed above. LPA met with Director Beth Hassenplug and explained the nature and purpose of the inspection. Director provided LPA a tour of the facility both inside and out. At the time of the inspection there were thirty-nine (39) children present and twelve (12) teachers.
The Department obtained an allegation that teachers were not providing adequate supervision by using cell phones for personal use during business hours.

LPA commenced interviews with both parents and staff members. During the course of the interviews, it was disclosed that cell phone use in a non-working capacity by staff members has occurred. Director stated that this is at times an issue and the facility is taking steps to remedy the issue.

This allegation causes a potential health and safety risk and based on interviews conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.
The following Type B citation for violation of Title 22 Division 12 101229 (a)(1) is being attached on the LIC9099D.
