LPA S. Mendoza-Ceja conducted an unannounced inspection to the center to complete the investigation of the above allegations. Investigation included interviewing the complainant, current/former staff, and some of the current/former parents of children in care, and reviewing records.

Staff interviews revealed the following: with staff #1 and staff #2 (refer to confidential names list).
In regards to Allegation #1, It was found that staff #1 had inappropriately yelled at child #2, another staff stated you could hear staff #1 yelling at the child #2 in the next room. It was also found that staff #2 inappropriately yelled at child #2 when she gets frustrated.
Allegation #2, Staff #2 inappropriately handled child #2 and #11 by grabbing them roughly squeezing them tightly at nap time. Allegation #3, It was also found at least child #2 and child #11 were sent to nap as a form of discipline.

Interviews were conducted with parents. Parent of child #2 stated child #2 would come home sad and had stated staff #1 yelled at him. One parent stated nap was used as a form of discipline for child #2.

LPA S. Mendoza-Ceja conducted an unannounced inspection to the center to complete the investigation of the above allegations. Investigation included interviewing the complainant, current/former staff, and some of the current/former parents of children in care, and reviewing records.

Staff interviews did not corrobrate the allegations of a child left unattended or being forced fed food.
It was found that the center had a no thank you bite policy and children are encouraged to try food.

Parent Interviews did not corrobrate the above allegations.

-Although these allegations may have occurred, there is not a preponderance of evidence to prove that the alleged violations did or did not occur, therefore, these allegations are deemed Unsubstantiated.
No deficiencies were cited. The LIC9213 (Notice of Site Visit) was posted during the visit.

Review of records revealed on 03/09/2018, staff #2 was observed yelling at a child and has inappropriately grabbed a child when a child was dumping sand on another child. In 2017, staff #2 was verbally reprimanded for a similar incident.

Based on LPAs observations, interviews which were conducted and record reviews, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED.


The following Type A deficiency is cited on page #3 according to CCR, Title 22 Division 12 Regulations in regards to Personal Rights. Appeal rights provided. Upon receipt, post and provide copies of this licensing report: to parents/guardians of children in care at the facility and to parents/guardian of children newly enrolled at the facility during the next 12 months. Licensee shall obtain signatures of parents/guardian on the Acknowledgement of Receipt of Licensing Reports LIC 9224.
