On 7/21/22, at 1:15 PM, Licensing Program Analyst (LPA) Elvin Baddley conducted an unannounced One Year Required inspection of the abovementioned Child Care Center (CCC). LPA met with Artesia Carlon, Program Coordinator of the CCC. LPA explained the purpose of the inspection to the Program Coordinator. LPA, in the company of the Program Coordinator toured the interior and exterior of the CCC. The CCC consist of a single classroom and an outdoor play area on the grounds the Clarence Ruth Elementary School. The CCC's hours of operation are from 8:00 AM -11:00 AM and 12:00 PM - 3:00 PM, Monday - Friday. As noted the CCC is closed from 11:00 AM -12:00 PM. No children are on site during the inspection.

The classroom used for child care has storage space ("cubbies") near the entry door for all children in care and age appropriate furnishings, toys and equipment. The CCC has a refrigerator in the CCC's kitchen and LPA observed a posted menu. LPA notes the kitchen is inaccessible to children in care. The CCC provides snacks to children in care and filtered drinking water is available for children inside and outside. Water fountains in the outdoor area are active, but are not used by children or school staff members. LPA informed Program Coordinator of the requirement to have CCC's water sources tested for lead containment. LPA observed required licensing notices and documents posted near the entry of the CCC. The facility uses an electronic application to sign children in and out of the CCC. LPA reviewed CCC Sign In/Out information and notes all information reviewed was found to be completed appropriately, having the authorized parent's electronic signature as well as entry and exit times.

LPA observed a carbon monoxide detector on site which was tested at 2:21 PM and found to be operable. Disinfectants/cleaning solutions and medications are stored in cabinets in the CCC's kitchen which is inaccessible to children in care. Sharps at the CCC are stored in the teacher desk which is inaccessible children.

The restroom of the CCC are observed to be void of toxins. Waste receptacles at the CCC are

observed to have tight fitting lids.

The outdoor playground area is enclosed by chain linked fencing and the entry/exit gates are secured. Furnishings, toys and equipment in the outdoor area is age appropriate. LPA observed two sheds in the outdoor play area which contain CCC supplies and equipment. The play structures in the outdoor play area have adequate cushioning materials (i.e wood chips) to absorb falls. Shade is provided by trees and building overhangs. Play structures are absent of sharp, pointed or loose parts. Program Coordinator is reminded to replace play equipment once the play equipment begins to degraded. No bodies of water were observed on site.

Staff and children records were reviewed. The children’s records were complete and found to contain emergency contact information as well as immunization records. Staff records were reviewed and contained current pediatric CPR and First Aid certifications (EMSA) along with Mandated Reporter certifications. Program Coordinator was reminded to keep training certifications current and updated prior to expirations. Program Coordinator notes no firearm or ammunition is stored on site. LPA notes the last fire/disaster drill in May of 2022 .

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility representative were reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Facility representative Artesia Carlon, .
