On 6/22/22, at 12:15 PM, Licensing Program Analyst (LPA) Elvin Baddley conducted an unannounced One Year Required inspection of the abovementioned Child Care Center (CCC). LPA met with Maria Beas, Site Supervisor of the CCC, and explained the purpose of the inspection. LPA, in the company of the Site Supervisor toured the interior and exterior of the CCC. The CCC operates out of one classroom on the grounds of Alvin Elementary School. LPA notes the preschool program share the building with an infant program ( toddler option) which has a separate operating license. The CCC's hours of operation are from 6:30 AM to 5:15 PM, Monday - Friday. During the inspection, LPA observed 16 children in care, along with four teachers providing supervision. All children were napping during the inspection and all staff members were wearing face coverings.

The classroom used for child care is organized and clean. The classroom has storage space ("cubbies") for all children in care and age appropriate furnishings, toys and equipment. The classroom has an operable refrigerator in the kitchen area which is inaccessible to children in care. The refrigerator contains food items consumed by the CCC's children which are labeled and dated. The CCC provided snacks, breakfast and lunch to children in care. There is available drinking water for children inside and outside. CCC allows the children to drink from on site water fountains and bottled water is provided. Site Supervisor was informed of requirement to have CCC's water sources lead tested. LPA observed a menu posted in the classroom. Additionally, LPA observed required notices and documents posted on the classroom's wall near the classroom entry door. LPA reviewed CCC Sign In/Out forms at the classroom entry and notes all forms reviewed were found to be completed appropriately, having the authorized parent's signature as well as entry and exit times.

LPA observed a carbon monoxide detector affixed to the classroom wall in the center. The detector tested at 2:50 PM and found to be operable. Disinfectants and cleaning solutions are stored in cabinet in the kitchen which is inaccessible to children in care. Sharps and medications are also stored in the kitchen area. LPA

notes medications in the CCC are secured in lock boxes atop the CCC's refrigerators.

The restrooms of the CCC are observed to be clean and void of toxins. Waste receptacles at the CCC are observed to have tight fitting lids.

The outdoor playground area is enclosed by chain link fencing. The fence’s entry/exit gates are secured and furnishings in the area is age appropriate. LPA observed locked sheds in the outdoor play area which contain CCC supplies and equipment. The play structures in the outdoor play area have adequate cushioning materials (i.e sand) to absorb falls. Shade is provided by shade covers and building overhangs. LPA observed water tables in the outdoor play area. Site Supervisor informed LPA the water tables are empty daily and children are directly supervised when engaged in play at the water tables.

Staff and children records were reviewed. The children’s records were complete and found to contain emergency contact information as well as immunization records. Staff records were reviewed and contained current pediatric CPR and First Aid certifications (EMSA) along with Mandated Reporter certifications. Site Supervisor notes no firearm or ammunition is stored on site. LPA notes the last fire/disaster drill occurred on 5/31/22.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Facility representative Maria Beas.
