25 | On October 21, 2022 at 12:03 PM Licensing Program Analyst (LPA) Rosie Breault made an unannounced inspection for the purpose of conducting a Case Management - Incident inspection. LPA conducted a Covid-19 prescreening and no exposure on site. LPA met with facility Director Susy Del Toro and discussed the purpose of the visit. LPA and licensee conducted a tour of the facility inside and out.
10/19/2022, director contacted Community Care Licensing (CCL) to self-report an incident of a child leaving classroom with teacher unable to stop child.
Per director, classroom where incident occurred is roughly 15 -20 feet from director’s office. Teacher Eliza had children at maximum ratio capacity (5). The child was close to door because was saying goodbye to a friend, while teacher had eyes on the child. Child then pulled on the handle and ran out. Teacher called for child to return and witnessed child run (indoor only) toward directors’ office – all within sight of teacher. Teacher could not go after as would leave her out of ratio. Teacher had eyes on child while she called for kitchen staff Sandy Lopez, who was nearby, and got child from office and escorted back to classroom. Incident lasted roughly 10 seconds. Teacher immediately met with parent at pick up, then teacher and director met with parents twice to discuss issues.
Per Title 22 Divison 12 101438.2 (d) a technical advisory was issued to facility and provided to director.
LPA advised possible new latch on door. Director stated to LPA she called handy man day of incident to install an additional safety latch. Now awaiting visit to install early next week.
