25 | Licensing Program Analysts, LPAs, C. Patterson and S. Mendoza-Ceja conducted an unannounced CASE MANAGEMENT inspection to the center regarding an incident that occurred on 09/20/2019 and was self reported to the Department as required. LPAs interviewed Director Susana Del Toro.
On 09/20/2019, Teacher #1 observed child #1 was running down a hill in the center's Pre K yard and slipped on mud at the bottom of the hill causing child #1 to fall onto the ground and bump her head on a cement walk. Center staff notified Child #1's parent/guardian. Child #1 was picked up from the center and received medical attention. Child #1 has returned to the center. At the time of the incident there was 1 teacher supervising 6 children.
LPAs toured the facility inside and outside. LPAs inspected the "muddy area" where the child had fallen. The Director stated the children were reminded where they can run and how to be safe. LPAs observed wood chips and cones placed in the area where it was muddy. Director stated additional dirt has been ordered and will be added to the area Director also stated that the grass is soon to be replaced with artificial turf and the irrigation system will be turned off. In addition, Child #1's file was reviewed.
There were no deficiencies cited during this inspection.
