The purpose of this amendment is to update the administrator's name.

On October 5th, 2022 at 2:10PM Licensing Program Analyst (LPA) Rosie Breault conducted a Required Annual inspection. LPA met with Director Jacqueline McDonough and explained the purpose of the inspection. LPA asked Covid-19 screening questions and no exposure on site. LPA with director, conducted a tour of the facility inside and out. There were seventeen (17) children in care at the time of the inspection and five (5) staff. The center operates from 7:30AM to 5:00PM and is open Monday thru Friday.

Licensing required notices were posted prominently at the front entrance and inside the front office. Center uses a written sign-in sheet. The facility uses two classrooms, and two bathrooms three (3) urinals and two (2) sinks with ample hand washing supplies. Bathrooms and classrooms were observed to be clean and inaccessible of toxins. Cleaning compounds are inaccessible to children and are maintained in a locked cabinet. There is water inside and outside for the children to have access to. Children bring their own water bottles and facility provides filter water. Classrooms have age appropriate toys and furniture available for children. The facility provides cots for napping and linens are kept separate from one another. The facility provides food to children and LPA observed the posted menu. There is a functioning carbon monoxide and smoke detector that meets statutory requirement. The last fire drill was conducted on 10/4/2022. First aid kits are available in classroom, labeled and inaccessible to children. Facility provides incidental medical services. Medications were properly labeled, dated to current, paperwork present, locked, and stored inaccessible to children.

The playground side area currently is sectioned off due to construction inaccessible to children. The remaining section of play yard has an ample amount of shade, hand washing station available, and age appropriate toys/equipment. Children are also utilizing the front yard for play time. Both areas have perimeter fencings.

A sampling of children’s files was reviewed and found current. Staff records are maintained at 2115 State Street location and LPA to review there. At this time, director who will remain present today, CPR expires 3/12/2024.

Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s record.
