The purpose of this amendment is to update the administrator's name.
On 04/16/2021 at 3:20pm, Licensing Program Analyst (LPA) S. Mendoza-Ceja conducted an unannounced tele-inspection via Facetime due to COVID-19 State of Emergency in order to conclude the investigation of the above allegation. LPA spoke with Alicia Jimenez, Site Supervisor and explained the purpose of the inspection. Site Supervisor and LPA conducted a virtual tour of the center. There were no children in care at time (children left 2:30pm). The allegation is that an unqualified staff is being counted in the ratio. Investigation included interviewing Executive Director, staff, and some of the parents of children in care; and reviewing staff records. LPA was unable to interview the complainant as no contact information was provided.
-Site Supervisor disagrees with the allegation. None of the staff interviews corroborated the allegation. None of the parent interviews corroborated the allegation. Records review reveals that all of the teachers are qualified and that some of the aides meet the teacher educational requirements. Although these allegations may have occurred, there is not a preponderance of evidence to prove that the alleged violations did or did not occur, therefore, the allegations listed above are deemed UNSUBSTANTIATED.
-Exit interview was conducted with Site Supervisor, via tele-inspection, during which appeal rights were explained. This report along with a copy of the appeal rights and Notice of Site Visit (LIC9213) will be sent to the Site Supervisor via email with a read receipt or confirmation of receipt of email, which will act as the Site Supervisor's signature. The LIC 9213 Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty.
