32 | LPA observed licensing required notices on the wall near the entry door of the CCC. LPA observed Sign In/Out forms on a clipboard near the CCC entry. LPA notes the forms contain the child's arrival and departure as well as the signature of the authorized representative/parent. Out of the twelve (12) infants and toddler present, two (2) were not signed in. Director immediately notified families to return to the facility to sign children in. LPA observed all families return to sign in their children.
The center’s outdoor play area is completely fenced. Entry/exit gates are secured. LPA observed the outdoor play area has concrete and grass area. LPA observed age appropriate toys/play equipment in the play areas. The toys/play equipment are free of sharp, loose or pointed objects. No bodies of water were observed on site.
Director notes no firearms/weapons or ammunition was stored on the premises.
A sampling of staff and children records were reviewed. The children’s records were complete and found to contain emergency contact information as well as medical assessments. Staff records were reviewed and contained pediatric CPR and First Aid certifications as well as Mandated Report certifications.
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
A notice of site visit was given and must remain posted for 30 days.
