1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 9/3/2021 @11:30 AM , Licensing Program Analyst Gigi Reyes conducted an unannounced Annua Required l Inspection, met with Licensee and discussed the purpose of the inspection. Prior to inspection, LPA asked pre health screening questions related to COVID 19, Licensee's responses indicate there are no COVID 19 exposure in the Family Child Care Home (FCCH). LPA and Licensee toured the interior and exterior of the home, there were 6 children present. During the tour LPA observed the following, required forms are posted in the prominent location. Appropriate fire extinguisher was last serviced on 6/30/2021. Smoke and carbon monoxide detectors were present. Toxins are locked, LPA observed clean, safe and age appropriate toys. The front and back porch are completely fenced, equipped with age appropriate toys and equipment. No bodies of water were observed on site. Licensee state there are no guns or ammunition. LPA reviewed the following documents, Pediatric CPR and First Aid Card expires on 1/13/2022. AB 1207 was taken on 3/28/2020. Immunization record for staff are complete. Fire and Disaster drill is conducted and logged every 6 months. Last drill was conducted on 6/2/2021. Children’s roster is current,

LPA reviewed children records. The records were current, complete and possessed emergency contact information as mandated by regulations. Licensee visually supervises and documents the sleeping positions and conditions of sleeping infants every 15 minutes.

The Licensee is not providing Incidental Medical Services (IMS). Policy was discussed. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA discussed COVID-19 best practices and guidance. Likewise, LPA discussed safe sleep regulations with the Licensee.

There were no deficiencies cited during this inspection.

Exit interview conducted with Licensee Maryvonne Lesvenan. Notice of Site Visit has been posted (LIC9213). The notice shall be posted for 30 consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty.
