32 | LPA observed a 7 month old infant in care (C1). LPA asked licensee for safe sleep log, LPA observed safe sleep log to be up to date. LPA asked licensee if infant sleeps in a crib or pin, licensee states neither. At 8:52 AM Licensee showed a picture of C1 sleeping on a mat fully swaddled with a blanket. The picture was dated 3/8/23. LPA issued a Type B citation for swaddling an infant and advised licensee swaddling is prohibited in day care homes. At time of record review, LPA did not observe the Individual Infant Sleep Plan (LIC9227) in the child's records. LPA explained and provided a copy of LIC9227 to licensee. Licensee is to send C1's completed LIC9227 by 5/8/23. LPA asked if licensee received PIN's, licensee stated yes. LPA provided licensee with a copy of PIN 20-24-CCP-SP Regarding Safe Sleep Regulations.
LPA reviewed five out of five children files. One out of five children's records was found to be missing immunization's. LPA issued a Technical Violation and reminded licensee that all children's records must be complete and up to date. Licensee CPR/first aid is valid through 10/2024. Assistant's CPR/ first aid certifications were also found to be valid. Licensee was reminded a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter certification AB1207 every two years at www.mandatedreporterca.com Licensee's mandated reporter certification was found to be valid through 9/2023. Assistant's mandated reporter certifications were also found to be valid. LPA reviewed assistant's files and found immunization records to be missing for both assistant's. Licensee states she must have misplaced them and states she will be sending proof of immunization's to LPA by 5/5/23. LPA issued a Technical Violation for missing immunization's records.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
