13 | On March 20th, 2023, at 12:06PM Licensing Program Analyst (LPA) Rosie Breault made an unannounced visit to conclude a complaint investigation that was initiated on 1/24/2023. LPA met with co-director Janeth Carachure and discussed the nature and purpose of the visit. Co-director provided LPA a tour of the facility inside and out. There were fifty nine (59) children care at the time of the inspection and nine (9) staff.
Allegation 1: Day care child was injured by another day care child.
The Department obtained an allegation a day care child was injured by another child. At initiation, director provided LPA a copy of the internal incident report, parent handbook, and other pertinent documents. An incident occurred on 1/13/2023 when two children were playing chase in the outdoor play area. One child (herein referred to as C1) ran by another child (herein referred to as C2) and scratched C2 on face, C1 then bit C2 on the back. Staff witnessed the incident, promptly separated, and redirected the children, and they later returned to normal play. Per handbook protocol, staff contacted both children’s parents informing them of incident and created an internal report. C1 was sent home and placed on a behavioral plan. Per Community Care Licensing (CCL) regulation, facility co-director reported incident to Licensing timely.
Based on LPA’s observation, record / file review, and interviews conducted, the preponderance of evidence standard for the entirety of the allegation has not been met, therefore the above allegation is found UNSUBSTANTIATED.
