On 04/08/22, at 11:40 AM, Licensing Program Analyst (LPA) Francisca Velazquez conducted an announced Prelicensing- change of location, change of capacity Inspection of the abovementioned home and met with Applicant, Laura Alapizco. Applicant previous facility number is 426215951. LPA informed Applicant of the nature and purpose of the inspection. LPA notes there are no children on site at the time of the inspection. Applicant informed LPA of the intention to maintain operating hours of a Family Child Care Home (FCCH) from, Monday- Sunday from 6:00 AM through 11:59 PM. The Applicant also Informed LPA of the intention to provide care for children birth to 13 years of age.

LPA toured the interior and exterior of the home with the Applicant. The home is a three (3) bedroom, two bathroom home. The home's living room, kitchen/dining area, restroom and outdoor yard will be used for child care services. Conversely, the home's bedrooms, garage and the side's of the outdoor yard will be excluded from child care. LPA observed the living room to have ample spacing and ventilation for children in care. LPA observed a fireplace in the living that is not made inaccessible. Licensee stated that a lock will be purchased to make sure the fire place is off -limits. During this inspection, Applicant ordered the fire place lock and showed proof to the LPA that Applicant will be picking up the fire place lock within the next two (2) hours. The home's bathroom to be used for child care is observed to be clean and free of toxins. LPA observed that sharps will be stores in an elevated cabinet in the kitchen. Cleaning compounds can be found in an elevated cabinet in the kitchen, in the garage and in an elevated cabinet in the hallway. All areas are off-limits to the children in care. Applicant stated that all household medication will be stored in Applicants bedroom that is also off limits to the children in care.

LPA observed a required fire extinguisher 2A10BC in the home which was purchased today, 04/08/22. LPA reminded Applicant of the responsibility to service or purchase a regulation fire extinguisher annually. The home has smoke and carbon monoxide detectors which were tested at 11:51 PM and found to be operable.

LPA and Applicant toured the outdoor yard. LPA notes that the outdoor yard is completely fenced. Applicant informed LPA of the intention of blocking off the side yards and making them off limits by means of fences to allow for active supervision. Applicant informed LPA that outdoor yard will have synthetic grass and a cement area for the children. Applicant informed that shade will be provided by means of tarps. Applicant stated that the tarps that are in her current home will be brought to the new home. Applicant informed that children will have access to drinking by means of water jugs and disposable cups.

LPA did not observe any children equipment, furnishing, toys in the home. Applicant stated that all furniture will be moved tonight and this weekend and will be available for the children on Monday 04/11/22. Applicant agrees to submit photographs of the home and day-care room set up.

LPA's record review revealed Applicant completed Preventative Health training on 02/21/2020. Further, Applicant completed Mandated Reporter training on 08/07/20, and Pediatric CPR/First Aid (EMSA approved) on 06/27/22. LPA reminded Applicant of obligation to maintain current training and certifications. LPA reviewed Applicant's control of property document (Basic Rental Agreement of Residential Lease). Applicant does not have liability insurance at the time but is in the process of getting an insurance policy for her day care. LPA provided Applicant with Affidavit Regarding Liability Insurance form (LIC 282) to be used until insurance policy is active. This home was granted fire clearance by Santa Maria Fire Department on 04/07/22.



LPA reviewed with Applicant the LIC 311D, Forms/Records To Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted.

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Exit interview conducted and report was reviewed with the Applicant, Laura Alapizco. The home does not meet Title 22 Division 12 requirements of a large FCCH license due to not having the home set up. Applicant was informed that family rooms and day care area need to be set up. Applicant informed LPA that she will close Saturday and Sunday to move household items and some day-care items. Applicant plans to operate in her current home on Monday 04/11/22 and would like for an LPA to visit on Monday 04/11/22 to grant her new large license and hopes to start offering day care services in this facility on Tuesday 04/12/22.
