The findings on June 22,2023, stated that an allegation was Unfounded and was found to be SUBSTANTIATED under Fixtures, Furniture, Equipment and Supplies 101239,(e)(4).
A NOTICE OF SITE VISIT WAS ISSUED, POSTED NEAR THE FRONT ENTRANCE TO THE CENTER, AND MUST REMAIN POSTED FOR 30 CONSECUTIVE DAYS.
