An unannounced Annual Random Inspection was conducted on this day, by Licensing Program Analyst (LPA) Almaraz, Celi. Facility is currently licensed for a capacity of 90. Licensing staff met with Director Smith, Carrie. The program currently operates 7 AM to 6PM, Monday thru Friday, these hours are for summer camp until 08/17/2021. Hours will resume to regular after this date. Licensing staff was taken on a guided tour of the facility of both indoors and outdoors. At the initial start of this visit, there is a total census of 37 children and a total census of six teachers present and Director.
PHYSICAL PLANT
This is a School Age program (summer camp) which consists of Portable E13 split into two parts SAC 2 and SAC 3. This facility is also licensed to use Portable 1 (Sac 1) of the Preschool program (434410779) which is closed due to San Jose Unified School District construction. All areas identified on the Facility Sketch were inspected. Upon arrival LPA Almaraz observed the following during this inspection of all rooms: Sac 1: Upon arrival, this room is closed due to construction. Sac 2: There were three staff (teachers) present and 16 children. There is one toilet, two sinks and one facet. Sac 3: There was two staff (teachers) present and 21 children. There is one toilet, two sinks and one facet.

Furniture and equipment were inspected for age appropriateness and are in good condition. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby and lockers to store their belongings. Per director, the isolation area is in the office. General sanitation was observed. Availability of indoor drinking water was observed in classrooms. Disinfectants, cleaning solutions, medication and other hazardous items that pose a risk to children, were inaccessible to the children in care. Director states that poisons are locked under kitchen sink.

LPA Almaraz observed the children's sinks are in working condition and meet licensing standards. LPA Almaraz observed soap dispenser and disposable towels in the bathrooms.

First Aid supplies were observed in the classroom in a cabinet, backpack and fanny packs. According to Director Smith, medication is only administered to a child when accompanied with a doctor's note and is stored in lockboxes in cabinet. Carbon monoxide detectors and smoke detectors are present and working in the facility. There is a Fire extinguisher size 3A40BC in each room and have been serviced on 06/07/2021. Teacher-child ratios were observed and staff names recorded. Last fire drill was on 05/18/2021.

Outdoor equipment was inspected for safety, cushioning material, good repair and appropriateness. Required shade, drinking water and fencing were inspected. LPA Almaraz advised Director Smith that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to drinking fountains during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed. Director states that this play ground is shared with the school district. Schedule differentials and supervision prevents commingling of children.



All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable trash receptacles have tight-fitting covers that are kept on, and in standard condition. Director states waste is to be discarded immediately after each meal. Snack/lunch menus were reviewed. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Food preparation areas were toured for safety, cleanliness and proper equipment. Breakfast is served daily and PM snacks, as well as lunch. Children can bring their own food if needed. Menus are posted one month in advance and posted in "parent" area. Menus available upon request for up to 1 year prior to today. All required postings observed. Refrigerator and room temperature meets licensing standards.

RAST: Children's temperatures are being taken upon arrival. LPA discussed the following: Disinfecting between use of lavatories and commonly touches surfaces daily or as needed. Social distancing during meals. Wearing masks. Hand washing. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. This facility is using Kinder Smart Electronic Application. THis is equipped for a full signature. Attendance sheets can be used if system errors occur.

Staff and Children’s Records were reviewed. Director was issued LIC 857 and LIC 859 for the review of child and staff records. Criminal Record Clearances were reviewed for adults, all adults have criminal record clearances. Inspection of required forms made. This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The following was discussed with the Director:
Rooms that are off-limits need to be made inaccessible during operating hours. Smoking is prohibited.
The fire extinguisher type 2A10BC or larger must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.
Mandatory Forms for the children’s files and staff files, requirements for fire drills, earthquake drills and documentation were discussed. Role and responsibilities of being a Mandated Reporter were reviewed. The Director was advised how to access forms and Regulations online at www.ccld.ca.gov. Director was made aware that it is his/her responsibility to know the regulations as well as anyone who assists in providing care. The Director was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check. LPA informed Director to log onto web site www.ccld.ca.gov to obtain forms and LIVE SCAN application. Records for all children and staff must be maintained for three (3) years after separation from the facility.The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation. Director advised that indoor and outdoor supervision required at all times. If outdoor area not adequately fenced, provider must be with children at all times when outdoors. San Jose Unified School Distrcit Construction of the school grounds has created geographical barriers for the preschool component and this facility. LPA observed that there is no safe way to access building (SAC 1) of the preschool, which was previously utilized as an isolation area and teachers bathroom. Director states that staff uses bathroom when children are not present (when children are outdoors), before and/or after school hours. In the event of a sick child in care they would isolate in the Directors office area and use one of the two bathrooms in this room, thoroughly cleaning after use. The San Jose Unified School District is currently closed (until 08/17/2021) therefore staff currently has no access to bathroom, which is in school building. In the event of a staff bathroom emergency, Director will ensure both supervision and personal rights of the children are being adhered to.
