On 3/6/24 Licensing Program Analyst (LPA) Sheena Chin conducted an unannounced Case Management- Licensee Initiated inspection. LPA met with the director, Elisa Hernandez and explained the purpose of this inspection. Present were the director, 3 teachers and 12 children in the facility. Staff records were reviewed. LPA toured inside and outside of the facility with the director. Required postings were observed. Ramps for easy access for disability children were observed in the front and back. .

On 12/5/23 the San Jose Regional Office received a 2 non-ambulatory slots application for Lyndale Head Start. Fire inspection was approved on 1/29/24. The director stated that no renovations were made to the facility for the 2 non-ambulatory slots application. The director stated that the facility would like to include more children with special needs and currently the facility had 3 preschool children enrolled, who had autism and speech difficulties. LPA reviewed the three children’s files.

The director stated that the three preschool children were from elementary school and joined the activities at the facility from 9:30am to 11:30am, 4 days per week. At 10am LPA observed that two of the preschool children joined the activities at the facility. The parent aid educator with the 2 children from the elementary stated that the other child was sick today. The capacity for the facility remained as 48. The director stated that the facility has allocated 4 spaces for children with special needs and 3 children with speech difficulties were enrolled. The director stated that the facility would like to add non-ambulatory services to the program and serve children with physical disabilities.

No deficiencies were cited. LPAs advised the director, Elisa Hernandez that a license for 2 non-ambulatory slots will be approved upon management review and approval.

Exit interview conducted and report was reviewed with Elisa Hernandez. A notice of site visit was given and must remain posted for 30 days.
