25 | On this day, an unannounced annual random Inspection was conducted by Licensing Program Analyst (LPA) Almaraz, Celi. This Facility is currently licensed for a capacity of 53; upon arrival there was a total census of 14 school age children with 2 teachers. LPA met with Director Adams, Rebecca. The hours of operation are 7:00 AM to 6:30 PM, Monday thru Friday (M-F), due to lower enrollment the correct hours are 8;30 AM to 6: 00 PM, M-F. LPA was taken on a guided tour of the facility of indoors/outdoors. This is a school age program which consists of one classrooms; The Pavilion. All areas identified on the Facility Sketch were inspected. This is a triple component facility, also licensed for Infant care # 434400312 and Preschool 434400313; the following was observed during the inspection of the facility:
PHYSICAL PLANT:
Pavilion: Upon arrival, had a total of 2 teachers with 14 shcool age children. LPA observed access to 2,
toilets, 3 sinks and drinking water. Teacher and child ratios were observed and staff names recorded.
Furniture and equipment: All furniture and equipment was inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Storage for children's belongings was observed. Availability of drinking water was reviewed. Toys observed to be clean. All storage containers for solid waste, including moveable trash receptacles have tight-fitting covers that are kept on, and in standard condition. Director states waste is to be discarded immediately after each meal. Director will replace any trash bins that do not have a foot pedal option.
Pavilion playground: Is in a safe condition, free of sharp, defective or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. There is adequate shade in the play yard. Availability of outdoor drinking water was observed. There is a concrete area where there is some root damage due an oak tree, there are some crevices in the area. Director will facilities maintenance to fill in the cracked areas. While this is currently not a hazard this is a preventative measure, issue will persist. 1/3 |
