Licensing Program Analyst (LPA) Samantha Yip conducted an unannounced Required 1 Year inspection. LPA met with Ms. Yolanda Sullivan, Licensee, and explained the reason for the inspection.

License, Emergency Disaster Plan, and Notification of Parent's Rights were posted. LPA reviewed in the sign in/out sheet, which contains full legal signatures. The hours of operation are Monday through Friday 7AM to 6PM.

LPA toured in the inside and outside of the facility. Furniture and equipment, such as chairs and tables, were age appropriate. LPA observed that there is sufficient amount of toys for children in care. Disinfectant, cleaning, supplies, and other items that are dangerous to children were inaccessible. LPA reminded Ms. Sullivan that all disinfectant, cleaning supplies, or other sharp objects, such as Lysol/Clorax wipes or disinfectant sprays, need to be stored inaccessible to children. The restroom for children was observed to be clean. LPA observed a fully charged fire extinguisher, smoke detector, and carbon monoxide detector. The last fire/disaster drill was conducted on 01/28/2020. Ms. Sullivan stated that there are no weapons, such as firearms, stored on the premise.

The outdoor area is fenced. Play structure was observed to be in good condition. Areas around play structure have resilient material to absorb falls. LPA reminded Ms. Sullivan to even out resilient material before children's use. Shaded rest area is provided through trees, canopy, and pergola. There were no bodies of water observed during today's inspection.

All meals are prepared and brought from home. Facility does provide snacks to children in care. Menu was observed to be posted. Area to store and prepare snacks was observed to be clean. Drinking water is provided through water jug and cups or individual water bottles.

Ms. Sullivan stated that she currently does have children in care who requires Incidental Medical Services. Medication is stored where it out of reach of children and with proper documentation. LPA reminded Ms. Sullivan to check expiration date on medication and that medication needs to be in the original packaging. LPA observed that a one child's medication was expired. Ms. Sullivan stated that she will obtain a new medication and send proof to licensing. 10 children's files were reviewed during today's inspection. The records reviewed include but not limited to admission agreement and immunization records.

4 staff files were reviewed during today's inspection. The records reviewed include but not limited to education credit and Mandated Reporter Training. LPA discussed with Ms. Sullivan that the Mandated Reporter Training requires renewal every 2 years.
LPA observed that S-1 and S-2 completed training in 10/2017 and have not renewed it. Ms. Sullivan stated that she will have S-1 and S-2 renew Mandated Reporter Training and send a copy to Licensing office.

All staff present have cleared criminal record and child abuse index clearance. LPA also reminded Director of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the license and who come in contact with or

provide care and supervision to the children. Penalty amounts: $100.00 per person per day, minimum of $100.00 to a maximum of $500.00 per person for an initial violation and a minimum of $100.00 to a maximum of $3000.00 per person for any subsequent violation within a 12 month period.

LPA discuss and provided Ms. Sullivan with Lead Exposure. Licensee is encouraged to visit the Department’s website at www.cdss.ca.gov to access resources for Providers, Title 22 Regulations, Online Licensing Forms, Adoption of new Laws, etc.

In the areas evaulated during today's inspection, two Type B deficiencies have been cited. An exit interview was conducted, where this report, the citation, plan of correction, and appeal rights were discussed with Licensee.
