Licensing Program Analysts (LPA's) Anna Morales and Mel Matos conducted a Case Management visit to issue deficiencies for an incident that occurred on May 15, 2023 and a change of the Child Director that had not been reported to Community Care Licensing. LPA's were greeted by Marcella Tarin and Mandy Hernandez.

Based on interviews and supporting documentation, it was determined that on May 15, 2023, approximately at 7:00am, parent(s) and staff were unable to enter the Center. To enter the Center, one must enter a code to unlock the door(s). Parent(s) contacted Jennifer Liu, District Leader, who gave Parent(s) a code via phone to unlock and open the door.

LPA's were informed that former Director Regina Swan's last day at center was on February 24,2023. This was not reported to Community Care Licensing within 10 days of the change.

Based on interviews with Director Marcella Tarin and Mandy Hernandez, a Type B citation was issued at today's inspection, and a copy of the Facility Evaluation Report LIC809/809-D. LPA's provided a copy of the appeal rights to Director, Marcella Marin prior to the conclusion of today's inspection.

Notice of site visit issued and must remain posted for 30 days.

This substitute child care center director shall be aware of center operations, including total enrollment; shall be trained in program operation; and shall be designated as an authorized person to correct operational deficiencies that constitute immediate threats to children's health and safety. This was not met by: On May 15, 2023, Staff who were present did not have the code to unlock the door and enter the preschool, which poses a potential risk to the health and safety of children in care.
