Licensing Program Analyst (LPA), Oscar Huang, conducted an unannounced Annual/Random inspection to the facility today. LPA met with Teacher Debbie McCarson and explained the nature of today's inspection to her. LPA toured the facility both inside and outside during today's inspection. LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC 610), Earthquake Preparedness Checklist (LIC 9148), Parents' Rights Poster (PUB 393), Personal Rights (LIC 613A), Child Car Seat Law (PUB 269), a monthly Menus and Activity Schedule. There are 3 classrooms licensed, rooms (PS-2 & 16B) are main classrooms for the preschool program, PS-1 is used for raining days, or special events. LPA observed 16 preschoolers with 2 teachers and 4 behavior specialists in classroom PS-2 and 11 preschoolers with 2 teachers in room 16B. The school offers 2 half-day sections: from 8am to 11am and 12pm to 3pm.

Staff of Almaden Preschool Program are hired through the San Jose Unified School District and fingerprint clearances are done through the Department of Education.



LPA reviewed six children and four staff files. Each child's file reviewed contains the identification and Emergency Information form (LIC 700), a copy of the admission agreement, Notification of Parent's Rights, and Personal Rights, and Physician Report. Immunization records are maintained and up-to-date. LPA observed that all children were properly signed in and out by an authorized representative electronically and in paper. All staff files reviewed contain the required transcripts/verification of experience. LPA observed all staff have current CPR and First Aid certifications on file. LPA observed that the teacher/child ratio was in compliance during today's visit. LPA observed the conditions, limitations, and capacity specifications of the Facility license are in compliance with the regulations, and children were visually supervised at all times.

LPA observed that all preschool rooms are clean and safe for all children and staff. Drinking water is readily available for the children in each room and outdoor via water dispenser. LPA observed solid waste containers with tight-fitting lids in each room. Staff and children's bathrooms are clean, sanitary, and operable. There were separate staff bathrooms in the office not utilized by the children which an isolated sick child can use if needed. Teachers stated that there are no weapons or firearms on the premises.

The facility provides breakfast for the AM section and Lunch for the PM section to the children. Food is prepared in the school central kitchen. LPA toured the kitchen, all food in kitchen are current, whole milk is provided. Children allergic food list are posted in each classroom for staff awareness. Cleaning supplies are inaccessible to the children and stored in high cabinets inaccessible to children in each of the classroom. Any medications at the facility are also stored in high cabinets inaccessible to children in each of the classroom. LPA observed all furniture and equipment is in good condition and safe for the children. LPA observed one functioning carbon monoxide in each of the classroom. The playground areas utilized by the preschool children is surrounded by appropriate fencing. LPA observed that the outdoor equipment is age appropriate and in good condition. LPA observed that there is grass around and under the play structures in the preschool playgrounds, shade is provided by building overhangs and tents. LPA did not observe any bodies of water in the playground area.

LPA observed staff have all the immunization records in file per SB 792 Immunization Requirements. Incidental Medical Services (IMS) were discussed. The facility provides Epinephrine Auto-Injector. IMS and IMS Service plan is on file. Currently, there was no children enrolled required IMS services. LPA discussed Beginning January 1, 2018, Health and Safety Code 1596.8662 requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Teacher, Debbie stated that all the staff have completed their training every year, the last was complete on Aug, 2019 and certifications are kept in each staff's file in the district office.

No deficiency was cited. LPA conducted an exit interview with Teacher, Cesar Herrera prior to the conclusion of today's inspection. A NOTICE OF SITE VISIT WAS ISSUED, POSTED NEAR THE ENTRANCE TO THE FACILITY, AND MUST REMAIN POSTED FOR 30 DAYS.
