25 | Licensing Program Analyst (LPA) Deanna Villagrana met with licensee Carmen Rosales for a required one year visit. LPA explained the nature of today’s inspection to her. Present were licensee, licensee's mother who is visiting from Mexico and one day care child. Days and hours of operation are Monday to Friday, 6:00am to 6:00pm. The adults that reside in the home are licensee, her husband, and adult son.
A review of staff records on 07/28/2021 indicates that all Facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Licensee understands upon notice of the Department to remove an individual from the home, or to exclude an individual from the home, the licensee shall immediately remove the individual and prevents them from returning to the home or having contact with children in care.
LPA toured the indoor and outdoor areas of the home during today’s inspection. LPA observed that the home is clean and orderly, with heating and ventilation for safety and comfort of the children. LPA did not observe stairs or a fireplace in the home. LPA observed licensee is using a converted garage for day care children. Licensee provided City of San Jose Permits for the converted garage. LPA informed licensee she may not use the room until in it approved by licensing. Licensee was informed of this on report dated 10/26/2018. LPA observed safe and sufficient materials, toys, and play equipment for the day care children. LPA observed cleaning compounds in a unlocked storage in the backyard and scissors in a kitchen drawer accessible to children. LPA observed a fully charged 2A10BC fire extinguisher. LPA observed a working smoke detector and a working carbon monoxide detector. Licensee states there are no weapons/firearms in the home. Off limit areas indoor: Master bedroom/bath, two bedrooms, and hallway closet. There are no bodies of water. Backyard is fenced. Off limits outdoor: right side of home that is fenced off to children and locked storage. LPA observed one dog and one cat in the home. Licensee states they are vaccinated. LPA observed licensee has a current CPR and First Aid certification expiring 08/10/2021 and completed Mandated Reporter training on 02/08/2020. |
