25 | Licensing Program Analyst (LPA) Elizabeth Berumen met with Licensee, Felicitas Rivera for an
unannounced Required – 1 year annual inspection. LPA was granted access to the home by the Licensee. LPA also observed six day care children (6 preschoolers) and Licensee's Assistant (Irma Tapia) in the home during today's inspection. Licensee was operating within the capacity and ratio requirements of her license. LPA observed the required postings, including the facility license, near the main entrance to the home. Days and hours of operation are Monday - Friday from 6:00 AM to 5:30 PM. Licensee states that herself, her husband, Nicolas Rivera Sr. and her daughter, Estela Rivera are the adults living in the home. There are no minor children residing in the home.
LPA reviewed a current Child Care Facility Roster and Fire/Disaster drill log during today's . The last fire/disaster drill was completed on September 13, 2023. LPA explained to her that a fire disaster drill was due on March 2, 2023. Licensee states that she has liability insurance for the day care thru DCI Insurance. Licensee has current CPR and First Aid certifications (expiration:06/05/2023). Licensee has the required vaccines (MMR, Tdap) and declines flu vaccine. Mandated Reporter Training for Child Care Workers (expiring 10/17/2024). LPA reviewed six children's files and the files were complete with the required forms and immunization records. LPA reviewed two staff files (Licensee and Assitant, Irma Tapia) and the file was complete with the required forms and immunization records.
LPA toured the on limit indoor and outdoor areas of the home during today's inspection. Licensee has a working telephone in the home (408-259-2064) and cell (408-890-8588). The home is clean, orderly, (including proper heating/cooling/ventilation), and safe for the day care children. There are safe & age appropriate toys, play equipment, and materials for the children in the home. LPA observed a wall heater that is not barricaded in day care room. LPA notes that that Felicitas uses a converted garage as day care room (fire marshal approval on 07/06/2020). Off limit area inside the home are 3 bedrooms and one bathroom. Licensee uses the kitchen/dining area as a passage for children to walk to the backyard.
LPA observed a fully charged 2A10BC fire extinguisher, working smoke/carbon monoxide detectors, no bodies of water, and fenced backyard. Licensee states that she does not have any weapons in the home. LPA observed 3 dogs in a gated area of the backyard. The garage is located in the backyard, and is off limits. LPA observed 5 locked storage sheds in the backyard. All detergents, cleaning compounds, medications, and other similar items are inaccessible to children. Licensee states that there are no poisons in the home. |
