25 | On 07/18/19 Licensing Program Analyst [LPA] Monica Mathur conducted an unannounced Plan of Correction [POC] inspection at St Francis of Assisi Preschool for citations issued on 07/15/19. LPA met with Lead Teacher, Irma Gonzales and explained the purpose of today's inspection. Irma stated Director Rochelle Leggitt took the day off and was not present in the Center. LPA called Director and informed her the purpose of today's POC inspection. Director gave consent for Irma to sign and receive the Facility Evaluation Report today.
On 07/15/19 Two citations were issued during an annual/random inspection, noted below:.
CCR 101170(e)(1) Criminal Record Clearance: Staff member Holly Tarver did not have criminal record clearance.
CCR 101216.2(e) Teacher Aide Qualifications & Duties: No fully qualified teacher was present and children were being supervised by an Aide and another staff (no ECE units) who did not criminal record clearance and was not associated to the facility.
On 07/16/19 Director submitted to CCL a written plan of corrections for above citations.
During today's POC Inspection, LPA inspected the Preschool room and observed 12 children bring supervised by 3 staff members- Irma Gonzales (fully qualified teacher), Daniel Vu (Aide) and Holly Tarver (no units) supervising the children. LPA checked the Personnel status and noted that Holly was cleared in the database, and all staff present in the room have criminal record clearances and were associated to the facility. Facility is in compliance with above regulations and citations issued on 07/15/19 were cleared during this inspection.
Exit Interview was conducted, where this report was reviewed and signed by the Lead Teacher, Irma Gonzales confirming receipt of documents. LPA also provided the Letters of Clearance. A NOTICE OF SITE VISIT WAS ISSUED, AND MUST BE POSTED ON OR ADJACENT TO THE INTERIOR SIDE OF THE MAIN DOOR INTO THE FACILITY FOR 30 CONSECUTIVE DAYS. |
