25 | Licensing Program Analyst (LPA), Janet Tse, conducted an unannounced annual inspection at the Facility today. LPA was greeted by site director Anna Kanapathipillai at arrival. Owner/Licensee came to the facility arround 2:00pm. LPA explained the nature of today's visit to both Licensee and Director. LPA toured the facility both indoor and outdoor. The Facility is licensed in the Two Years Old Room, Three Years Old Room, and Four Years Old Room. LPA observed the facility sketch does not accurately depicts the entrance of each classroom. LPA also observed some classrooms have a dividing wall in between.
LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC 610), Earthquake Preparedness Checklist (LIC 9148), Parents' Rights Poster (PUB 393), Personal Rights (LIC 613A), Child Car Seat Law (PUB 269), Menus (includes current and following week), and Activity Schedules. The Facility has an active waiver for outdoor playground space.
A listing of staff criminal record clearances associated to this facility in the CCL Licensing Information System (LIS) on 03/09/2020 was reviewed; and it indicates that all Facility staff or other individuals who require caregiver background clearances have received criminal record and child abuse index clearances or exemptions.
LPA reviewed ten children's and three staff files during today's visit. Each child's file reviewed contains the Information and Emergency Information form (LIC 700). LPA observed the site director doe not have the transcript for the administrative class in file. LPA observed at least one staff on site has current CPR/1st Aid certifications in file. Licensee understands that there shall be at least one person, with valid CPR and First Aid certifications, on site at all times or present during off-site activities (field trips).
LPA observed the teacher/child ratio was in compliance during today's visit. Licensee understands the conditions, limitations, and capacity specifications of the Facility license. Licensee understands that children
