Licensing Program Analyst (LPA), Fermin Campos-Jaramillo, met with Elizabeth Ceasar, Site Director, for an unannounced annual random inspection. LPA toured the indoor and outdoor space during today's inspection. LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC 610), Earthquake Preparedness Checklist (LIC 9148), Parents' Rights Poster (PUB 393), Personal Rights (LIC 613A), Child Car Seat Law (PUB 269), Menus, and Activity Schedule. The Facility has one active wavier for use of one preschool bathroom for isolate the ill children. LPA observed the sign in/out sheet is complete. LPA obtained a copy of the current children's roster.

A review of staff records on 5/07/2019 indicates that all Facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. LPA reminded the Director of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the license and who come in contact with or provide care and supervision to the children. Penalty amounts: $100.00 per person per day, minimum of $100.00 to a maximum of $500.00 per person for an initial violation and a minimum of $100.00 to a maximum of $3000.00 per person for any subsequent violations within a 12 month period.
LPA reviewed a sample of ten children's and eight staff files during today's inspection. Each child's file reviewed contains the Information and Emergency Information form (LIC 700), a consent for Emergency Medical Treatment form LIC627, a Acknowledgement of Notification of Parent's Rights LIC995, and Immunization Records Form PM286. All teacher files reviewed contain the required transcripts/verification of experience. LPA observed at least one teacher have current CPR and First Aid certifications on file. The Director understands that there shall be at least one person, with valid CPR and First Aid certifications, on site at all times or present during off-site activities (field trips). LPA observed that none of the teachers and helpers have completed the mandated reporter training according with the AB1207.

On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following

Site Director shall obtain proof of completion of the mandated reporter training for herself and for all the staff members, according with the AB1207. Site Director will submit a copy of the certificates to Licensing Department on or before June 24, 2019.

This regulation was not met as evidenced by: LPA observed that none of the members of the staff have certificate of completion of the "mandated reporter" training (AB1207) Licensee understands this poses a potential risk to the children in care.

LPA observed that the teacher/child ratio was in compliance during today's inspection. The Director understands the conditions, limitations, and capacity specifications of the Facility license. The Director understands that children shall be visually supervised at all times. LPA observed the Director, and eight teachers providing care to 55 preschool children in the Facility during today's inspection. The Director states that the Facility does not provide transportation.
LPA observed that all rooms are clean and safe for all children and staff. Drinking water is readily available for the children in each room and in the outdoor playground area via water bottles. LPA observed solid waste containers with tight-fitting lids in the Facility. Staff and children's bathrooms are clean, sanitary, and operable. There is a separate bathroom for the staff in the other wing of the building. The Director states that there are no weapons or firearms on the premises. The Facility has functioning carbon monoxide detectors and a system of smoke detectors and alarms.
The Center provide snacks to children and the snack preparation and storage areas are clean, free of litter & rubbish, and free of rodents and other vermin. LPA observed the snack preparation area is equipped with refrigerator, microwave oven, stove, and sink with running water. All food and beverages that require refrigeration are stored in covered containers at 45 degrees F or less. Cleaning supplies are inaccessible to the children. The school does not administer medication except for Incidental Medical Services Plan which has been submitted before and is active. Any medications at the Facility are stored appropriately inaccessible to children. LPA observed all furniture and equipment is in good condition and safe for the children. The playground areas utilized by children are surrounded by appropriate fencing and the outdoor surfaces are safe for the children. LPA observed trees and tarps provide shade to the playground area. LPA observed that the outdoor equipment is age appropriate and in good condition. There is rubber mulch and wood chips as resilient materials under the structures in the playground areas. LPA did not observe any bodies of water. LPA provided the Site Director with the Department website address for information and completion of the "mandated reporter" training AB1207 www.ccld.ca.gov

One Type B deficiency was issued during today's inspection.
LPA conducted an exit interview with the Director, and Licensee rights form was printed and handed to the Director.
A NOTICE OF SITE VISIT WAS ISSUED, POSTED NEAR THE FRONT ENTRANCE TO THE FACILITY, AND MUST REMAIN POSTED FOR 30 DAYS.
