Licensing Program Analyst (LPA) Stephanie Collins conducted an unannounced Annual Inspection of the Preschool. LPA met with Director Bizhu Luo and explained the purpose of the inspection.

Facility’s License, Parents’ Rights Poster, and Activity Schedules were observed to be posted. Facility's operating days and hours are Monday to Friday 8:30 AM to 6:00 PM. LPA inspected the building inside and out with Director. There were 25 preschool children and five staff supervising the children, Facility was observed to be in compliance with teacher to children ratio requirement during the inspection. Children were engaged in activities under the visual supervision of the teacher.

Facility representative Bizhu Luo was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Facility has two classes with the following names: (1) Bumblebee, and Ladybug. Facility's classroom, play area, restrooms, kitchen, storage area, and office room were inspected. Director stated that facility does not possess nor store any weapons on the premises. Disinfectants, cleaning solutions, and other items that are dangerous to children were stored inaccessible to children. Furniture and equipment such as mats, cots, tables, and chairs were age appropriate and were in good condition, free of sharp, loose, or pointed parts. Restrooms for children's use was observed to be in safe, sanitary, and operating condition. Floors were clean and free from tripping hazard.

Outdoor activity space is fenced and play equipment were maintained in a safe condition and free of hazards. There were no bodies of water observed. Areas around and under high climbing equipment and slides were cushioned with material that absorbs falls. There were shaded rest areas for children. Drinking water are arranged to be readily available to children during indoor and outdoor activities.

Facility only provides snack. Trash cans for solid waste had tight-fitting covers on and were in good repair. Currently, facility does not provide transportation for children, but facility understands that children cannot be left alone, unattended in park vehicles. First Aid Kit was inspected. Fire extinguisher fully functional. Smoke and Carbon Monoxide Detectors were observed. Carbon Monoxide Detector was tested and proved to be functioning. LPA observed that the facility has a fire alarm pull station. Fire and Evacuation drills were last conducted on 08/30/2022. Director states there are no weapon stored in the facility.

Facility files were reviewed including the children sign in and out procedure. A sampling of Children files was selected for review. Records reviewed include Admission Agreement, Identification and Emergency Information, Medical Assessment, Consent for Emergency Medical Treatment form and Immunization.

Five staff files was selected for review. The records reviewed include: Criminal Record and Child Abuse Index Clearance, Health Screening Report with TB Clearance, immunization record, training such as the Online Mandated Reporter Training, and Education Qualification. There was at least one staff with current certification in Pediatric CPR and First Aid present during the inspection.

Beginning January 1, 2018, Health and Safety Code 1596.8662 requires all licensed providers, applicants, directors, and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. The training requirement may be met by using the Department’s Office of Child Abuse Prevention (OCAP) online training modules. The OCAP modules are free of cost and available at:

Therefore, citations was issued. Exit Interview was conducted, where this report, the citation, plan of correction, and appeal rights were reviewed and discussed with Director Bizhu Luo.
A notice of site visit was given and must remain posted for 30 days.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
