32 | there was no one in the home. Licensee stated she said she was not in the home. LPA asked why her mother in law did not allow LPA in the home. Licensee stated she does not allow her assistant to open the door for anyone. LPA was not granted entrance at time of arrival. After interview and investigation, Licensee's mother in law was left alone with four infants and two toddlers.
LPA toured the indoor and outdoor areas of the home during today’s inspection. LPA observed the carpet in the day care room needs to be clean. LPA observed barricaded fireplace inside the home. LPA observed safe and sufficient materials, toys, and play equipment for the day care children. All sharp objects, detergents, cleaning compounds, medications, poisons, and other similar items inside the home are stored inaccessible to children. LPA observed a fully charged 3A40BC fire extinguisher, a working smoke and carbon monoxide detector. Licensee states there are no weapons/firearms in the home. Off limit areas indoor: master bedroom/bath, three bedrooms, kitchen, living room and dining room. There are no bodies of water. Backyard is fenced. Off limits outdoor: the detached office unit, the fenced area next to the office unit, the front area of the right side yard, and the back area of the left side yard. LPA notes there is a side gate that leads to the ADU that is registered as 4817 Alum Rock Ave., San Jose, CA 95127. Licensee has two dogs and are vaccinated. LPA observed licensee and her mother in law have a current CPR and First Aid certification expiring 05/06/2023 and both have completed Mandated Reporter training.
LPA did not observe a current roster of the children. LPA observed a fire and disaster drill log was last completed on 08/03/2023. LPA reviewed 11 children's files. Child 2, 3, 6, 7, 8, and 9 are missing LIC995. Child 5 immunization records need to be updated. Child 4 is missing LIC9227. Licensee states she does not have a Safe Sleep log for infants in care. Licensee states day care is insured with Farmers and expires 03/01/2024. LPA discussed SB792 Immunization Requirements and observed licensee and assistant have immunization records on file.
Supervision of children was discussed with licensee and she understands that she must be present in the home during day care hours and ensure that the children are supervised at all times. Licensee understands her capacity options and she understands that she cannot have more than 14 children in the home at any time. Licensee understands if she transports children via vehicle, children cannot be left in parked vehicles unattended at any time. |
