Licensing Program Analyst (LPA) Pete Hernandez conducted an unannounced subsequent complaint visit today. LPA met with Licensee, Aljbana Kraja.
On 11/01/2022 at 2:00 pm, LPA conducted an initial complaint investigation of the above allegations and observed 9 (C1-C9) children in care. The licensee and 2 assistants (S1-S2) were present. LPA arrived during nap time and observed C1 and C2 sleeping in car seats. No children were observed swaddled.
Today, LPA arrived at the facility at 11:30 AM entered the facility at 12:45PM and observed 8 children in care (Ch1-Ch8) (2 preschoolers and 6 infants), Licensee and helper.
LPA reviewed children’s records. On 11/3/22: Based on records: childrens files and children's roster, children (Ch1-Ch6) were under the age of 2. The facility is licensed to serve a maximum capacity of 4 infants (under 2 years of age) even when there is an assistant present. No exceptions have been granted to the facility.
Based on LPA’s observations, records reviewed, and interview with licensee, the preponderance of evidence standard has been met. Therefore, the above allegations are SUBSTANTIATED. Deficiencies are being cited today.

There is an issuance of Type A citations, a copy of the Facility Evaluation Report LIC809 has to be posted on the wall and a copy to be given to all parents of currently and newly enrolled children for next 12 months. In addition, copy of LIC9224 Statement Acknowledging Receipt of Licensing Reports need to be signed and kept in child files.

An exit interview was conducted with licensee, Aljbana Kraja, where this report and licensee’s appeal rights were provided and discussed.

A NOTICE OF SITE VISIT WAS ISSUED, POSTED NEAR THE ENTRANCE TO THE FACILITY, AND MUST REMAIN POSTED FOR 30 DAYS.
