Licensing Program Analysts (LPAs), Cortney Nelson and Mel Matos, met with Tiffany Brunnings, Executive Director, and Julie Dingmann, Infant Director, for an unannounced Required- 1 Year inspection (Kit 2). LPAs notes that the Facility is located on the campus of Cisco Systems and entrance to the Facility is via the left side section of the parking lot. The infant program is licensed in Rooms A-G and 1-7 plus the infant/toddler park. The Facility is currently not using Rooms A, C, D and Rooms 2, 3, 6, & 7 due to low enrollment. The Facility also has a licensed preschool program (#434410601) and school age program (#434410602) on the same campus. Hours of operation are temporarily adjusted to: 8 AM to 6:00 PM.

LPAs toured the Facility both indoor and outdoor areas of the Facility with Tiffany and Julie during today's inspection. LPAs observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC 610), Earthquake Preparedness Checklist (LIC 9148), Parents' Rights Poster (PUB 393), Personal Rights (LIC 613A), Child Car Seat Law (PUB 269), Menus, and Activity Schedule.

A review of staff records on October 5, 2021 indicates that all Facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. LPAs reminded Tiffany of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the license and who come in contact with or provide care and supervision to the children. Penalty amounts: $100.00 per person per day, minimum of $100.00 to a maximum of $500.00 per person for an initial violation and a minimum of $100.00 to a maximum of $3000.00 per person for any subsequent violations within a 12 month period.

LPAs reviewed eleven children's and 21 staff (19 Teachers & 2 Aides) files during today's inspection. Each child's file reviewed contains the Information and Emergency Information form (LIC 700) and Needs & Services Plans. Needs & Services Plans also include individual feeding plans for each child and are updated at least once per quarter or more often if needed. All staff files reviewed contain the required transcripts/verification of experience/ immunization records and Health Screening Report.

All infant staff have completed the required Mandated Reporter training for Child Care Workers. All infant teachers have completed the required infant/toddler course. There is at least one staff in the Facility with valid CPR & First Aid certifications on file. Tiffany understands that there shall be at least one person, with valid CPR and First Aid certifications, on site at all times or present during off-site activities (field trips). LPAs observed that the teacher/child ratio was in compliance during today's inspection. Tiffany understands the conditions, limitations, and capacity specifications of the Facility license. Tiffany understands that infants shall be visually supervised at all times and never left unattended, including the crib areas.

LPAs observed that all rooms are clean, safe, sanitary, and in good repair for children, staff, and visitors. The Facility uses a third party cleaning service that cleans the Facility after hours Monday to Friday. The Facility has central heating/air throughout the Facility. Drinking water is readily available for the children in the infant rooms and in the outdoor playground areas via sippie cups, drinking fountains, and water faucets/disposable cups. LPAs observed solid waste containers with tight-fitting lids in each room. The staff bathrooms are clean, sanitary, and operable. There are separate staff bathrooms not utilized by the children which an isolated child can use if needed. Julie states that there are no weapons or firearms on the premises. The Facility has functioning carbon monoxide detectors throughout the Facility.

The food preparation and storage areas are clean, free of litter & rubbish, and free of rodents and other vermin. All food and beverages that require refrigeration are stored in covered containers at 45 degrees F or less. Cleaning supplies are inaccessible to the children and stored on upper shelves or locked janitorial closet.

LPAs observed that all furniture and equipment is in good condition and safe for the children. The infant changing tables have raised sides that are at least 3 inches in height. The infant changing tables have vinyl changing pads that are at least 1 inch thick. Napping equipment (mats/cribs) is appropriate. The Facility was in compliance with the Safe Sleep regulations/requirements, including the Infant Sleeping Plan (LIC 9227).

The playground areas utilized by children is surrounded by fencing and the outdoor surfaces are safe for the children. Sufficient shade is provided by trees, umbrellas, and building overhangs. LPAs observed that the outdoor equipment is age appropriate and in good condition. There is sufficient resilient materials (rubber surfacing) in the outdoor playground area. LPAs did not observe any bodies of water during today's inspection.

LPAs conducted an exit interview with Tiffany and advised her that no deficiencies were issued during today's inspection.
A NOTICE OF SITE VISIT WAS ISSUED, POSTED NEAR THE ENTRANCE TO THE FACILITY, AND MUST REMAIN POSTED FOR 30 DAYS.
