On 12/8/2021 approximately 8:45am Licensing Program Analyst (LPA) Kelly Phan conducted an UNANNOUNCED ANNUAL REQUIRED INSPECTION. LPA Phan met with Sepideh Sotoudeh (Facility Director), also present at the time of this inspection were 4 staff members and 2 infants and 7 toddlers . The facility is within ratio and capacity compliance today. The facility was toured to conduct a Health and Safety Inspection. This program current operates Monday-Friday 8:30am-4:30pm out of an Infant/Toddler classroom.
The classroom were inspected for age appropriate furnishings, equipment, and adequate storage for children’s belongings. LPA observed the cleanliness of floors and surfaces, the presence of working carbon monoxide detector, smoke detectors/fire alarms and a fully charged fire extinguishers size 2A10BC that is accessible throughout the facility. There are changing tables within arm’s reach of the sink; each bathroom was inspected and had good supplies of paper towels and toilet paper with working toilets. The outdoor play area is fully fenced with full supervision. There is padding under/around the climbing equipment. LPA observed the play structures to be fully intact with no visual defects or concerns. There are no pools, hot tubs or other accessible bodies of water. Per director, there are no firearms present on the premises. Cleaning supplies/toxins are stored inaccessible to children during the inspection and all required postings are present. Per director, parents provide snacks/meals for the children due to COVID. LPA reviewed storage of food and medications are all stored in their original packaging and labeled with expiration dates and child's names. The children's medications were also inspected and were founded to be in good condition and documented correctly. The center also has documents of fire and earthquake drills conducted once every 6 months and the logs show that drills were conducted on 09/23/2021 by City of Mountain View Fire Marshall.

At 12:00pm, 6 children files and three staff files were reviewed and complete and found to be fully qualified infant teachers, but one file had an incomplete LIC 700 that was missing a signature and was completed by today. The sign in/out binder was reviewed and 9 signatures present for 9 children. A copy of the facility personnel roster and children’s roster were available for review and copy was obtained. All staff members have completion of Mandated Reporter training as well as proof of immunizations. All staff have current CPR/First Aid certificates, and proof of education and/or experience was also present. LPA advised all staff members and director to not have any mobiles or toys inside the infant cribs. Director and staff members understood and will immediately take it down as of today. LPA also gave LIC 9227 and sleep log to the director to use for future references. The first aid backpack was observed and found to be complete.

Exit interview conducted. Appeal rights and notice of site visit was given to director to post for 30 days. There were no deficiencies cited as of today's inspection.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
