On Friday, June 3, 2022 at 9:15 AM, Licensing Program Analyst (LPA) Manel Estoesta conducted an unannounced Required 1 Year Visit. LPA met with the Director Rupa Sethna and explained the nature of the visit. Present on this visit were 2 Staff and 12 preschool children. Facility operates from Monday to Friday 8 am to 6 pm.

LPA toured the facility to conduct a health and safety inspection with the Director. There are two active classrooms namely as Shiny Stars (ages 4-5) and the Little Flowers (ages 2-3). The classrooms were age appropriate and in good repair. The kitchen area was maintained in a clean manner and was inaccessible to children in care. Bathrooms were clean and in working order. There is a separate staff bathroom. Snacks are provided and prepared on site. Parents provides lunch for the children. Waste containers have tight fitting lids. The storage of napping equipment was observed. The sign in and out logs were reviewed. Firearms and other weapons are not being stored on the premises. All posting requirements are being met. Outdoor play area was free of hazards and provided a shaded area for the children and access to drinking water. Playground equipment is cushioned with material that absorbs a fall. Play yard were age appropriate and in good repair. There are no bodies of water. The facility has a carbon monoxide detector, charged fire extinguishers and first aid kits. LPA reviewed the Milpitas Fire Department Permit that expires 12/31/2022. Fire and disaster drills are being conducted monthly. There is a working telephone at the facility.

Facility had a Water Lead Testing conducted on 08/13/2021 and LPA reviewed Certificate of Analysis with the Director.

Personnel files were reviewed today. The teacher/child ratio is being met. A percentage of children's files were reviewed at 10 am. Roster of the children and staff is maintained, and LPA obtained copies.

The Director and Staff CPR and First Aid certificate are current. The Director and Staff completed the Mandated Reporter General Training and Child Care Providers training online at https://mandatedreporterca.com/ are current. Director and Staff have records of Measles and Pertussis immunization, Influenza vaccination and TB clearance. LPA reminded the Director that only the Influenza vaccination can be decline with a written declination.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

There are no deficiencies cited on this visit.

A notice of site visit was given and must remain posted for 30 days.
