Licensing Program Analysts (LPAs) Anna Morales and Farida Raja conducted an unannounced Required - 1 year inspection and was greeted by Sandra Choi, Director. The Facility is licensed in the main room, busy bee/play rooms, and dance room. Hours of operation are 8:30 AM to 5:30 PM Monday to Friday. Facility License, Emergency Disaster Plan (LIC 610), Earthquake Preparedness Checklist (LIC 9148), Parents’ Rights Poster (PUB 393), Personal Rights (LIC 613A), Child Car Seat Law (PUB 269), and Activity Schedules are posted on a poster board Last disaster/earthquake drill was conducted on 1/17/2023.

LPA 's observed Director, one teacher and one teacher's aide with a total of 13 children(24 months to preschool). The teacher/child ratio was in compliance during today's inspection.
LPA's observed cubbies for each of the children's personal belongings. LPA's observed age appropriate furnishings. Director stated that they provide breakfast/brunch, lunch and two snacks. Menus are posted and sent to the parents(via email). The trash bins have tight fitted lids. All of the disinfectants and toxic materials are stored inaccessible to the children. Observed two fully charged fire extinguishers. one in the front area and the back area. The Carbon monoxide is interconnected with the Emergency Fire Alarm. Parents use a electronic sign/sign out. Outdoor activity space is enclosed by fencing and is observed to be free of hazards. LPA's observed play equipment were in good condition. LPA's observed resilient materials under the climbing structures. Drinking water was readily available to children indoor and outdoor.

This facility is not providing Incidental Medical Services – IMS Plan. None of the children currently enrolled are using medication. Director stated that if a child gets sick, there is an isolation area for the child until pick up. LPA's reviewed a random selection of children files. Children records reviewed include Admission Agreement, Identification and Emergency Contact, Consent for Emergency Medical Treatment form, receipt of Parent Rights Notice, Personal Rights Notice, Medical Assessment, Immunization.

LPA's reviewed Staff's records reviewed include Criminal Record and Child Abuse Index Clearance, Health Screening Report and TB test, Immunization (Measles, Pertussis, and Flu) record and required Training. S3 did not have a complete Personnel record on file, including Tb clearance and Immunization's.
LPA's reminded Director that the online AB1207 Mandated Reported Training needs to be renewed every two years. All of the staff's ( Director, S1 and S3) did not have current Mandated Reporting Training. There was at least one person with current certification in Pediatric CPR and First Aid present at the facility.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA's discussed the requirements of AB 633 with the Director and understands the AB 633 fact sheet/copy of Acknowledgement of Receipt of Licensing Reports (LIC 9224). LPA's discussed "zero tolerance" related regulations and advised that the assessment of an immediate $500 per day civil penalty for any violation of a "zero tolerance" related regulation. An ongoing civil penalty of $100 per day continues until the violation(s) is corrected.

Exit Interview was conducted with Director. Deficiencies are being cited based on the LPA's observations, interviews conducted and records reviewed in accordance with the California Code of Regulations Title 22.

A NOTICE OF SITE VISIT WAS ISSUED, POSTED NEAR THE ENTRANCE TO THE HOME, AND MUST REMAIN POSTED FOR 30 DAYS.
