On 04/27/2022 at 9:30am, Licensing Program Analysts (LPAs) Christina Uribe and Morgan Pringle conducted an Unannounced Annual Required Inspection. LPAs met with Licensee, Haidee Lim also present at the time of the inspection were 9 staff & 24 children. The facility is within ratio & capacity compliance today. LPA provided facility representative the Entrance Checklist (LIC 125). The facility was toured to conduct a Health and Safety inspection. The facility currently operates 7:00am-6:00pm, Monday-Friday in 3 classrooms. The facility is located on the Immanual Lutheran Church property.

Classrooms: All classrooms were inspected for age-appropriate furnishings, equipment, & adequate storage for children’s belongings. LPA observed the cleanliness of floors & surfaces, the presence of a fully functional carbon monoxide detector, smoke detector/fire alarms, and a fully charged 3A40BC fire extinguisher that is accessible throughout the facility. The changing tables are clean and within arm’s reach of a functional sink. The center is equipped with a fully stocked first-aid kit and available in the classrooms. LPA Pringle observed a baby rocker in the infant classroom and informed the licensee of the regulation of this banned item in child care centers.

Safe Sleep: All cribs were inspected for model compliance, inside of crib consisted of a fitted mattress and sleeping infants were covered with a blanket and five cribs had mobiles attached to them. LPAs observed Individual Infant Sleep Plan for all infants under the age of 12 months. LPAs reviewed sleep record log for 15-minute checks and found that the facility only documents this for infants 12 months or younger. During the tour, LPAs observed two infants under the age of 12 months placed on their stomachs to sleep and record review of sleep log shows that infants of this age range are regularly placed on their stomach to go to sleep.

Food Service Areas: All children’s individual food items are labeled and properly stored. All center provided food items are properly labeled & stored separately from cleaning supplies.

Outdoor Play Area: There are no bodies of water, or free-standing water accessible to children. There are age appropriate toys and materials for the children. The playground outside is fenced and all equipment and surfaces are free from hazards. Facility does have a waiver for the outdoor playground which is posted as required.

Records: All individuals subject to criminal record review have a clearance and have been associated to the facility. LPA reviewed 11 children’s files and 10 staff files around 11:30am. 4 staff files and 2 children's files need updated immunization records. Needs & Services Plan for children need to be updated quarterly. LPA reviewed the facility roster & personnel record. At least one opening/closing staff member has a current Pediatric CPR/First-Aid Certification. Mandated Reporter certificates were reviewed and 3 staff members do not have a current certificate. Director’s CPR/First-Aid Certificate is current and expires on 09/29/2023. The center is in compliance with sign in and out procedure. Emergency Drills are recorded and performed at least every six months. Per facility representative, there are no firearms on the premises. All required documents are posted in a publicly accessible area.

Health-Related Services: This facility does provide Incidental Medical Services (IMS). LPA inspected storage of medications and equipment/supplies, and reviewed children’s, personnel, and administrative records. Two children have prescription and nonprescription medications at the facility both of which do not have a signed Parent Consent for Administration of Medication and Medication Chart (LIC 9221) form. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Effective August 1, 2003 California Law requires Child Care Licensees to report unusual incidents or injuries to children in care to child’s parents and to the Department of Social Services using the Unusual Incident/Injury Form (LIC 624). Incidents must be reported within 24 hours to the regional office by phone and the written report, LIC 624, within 7 business days.

LPAs Uribe & Pringle informed licensee, Haidee Lim, that this report dated 04/27/2022 document one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.

Also, LPAs Uribe & Pringle informed the licensee to provide a copy of this licensing report dated 04/27/2022 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the licensee, Haidee Lim.
