Licensing Program Analyst (LPA), Oscar Huang, conducted an unannounced annual/random inspection to the facility today. LPA met with Director, Lili Zhu, and explained the nature of today's inspection to her. LPA toured the facility both inside and outside during today's inspection. LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC 610), Earthquake Preparedness Checklist (LIC 9148), Parents' Rights Poster (PUB 393), Personal Rights (LIC 613A), Child Car Seat Law (PUB 269), Menus includes current and following weeks and Activity Schedule. Also, the facility has and posts a waiver on playgrounds usage. LPA observed 29 preschool children in 3 classroom with with total 6 teachers (2 teachers per classroom).

Walk Through Indoor: LPA observed no bodies of water indoor. Director states there are no weapons at the center. All children are under supervision, including visual supervision during the visit; and the facility is operating within ratio. Temperature was measured between 70 and 72 degrees F by thermostats on the walls. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored inaccessible to children. Storage room for poisons are locked outside the building. All materials and surfaces accessible to children are toxic free. Medication is stored in a lockbox in the top cabinet in each classroom inaccessible to children. Children’s toilets and hand washing appears in safe and sanitary operating conditions. There is a separate staff bathroom in front desk not utilized by the children which an isolated child can use if needed. Floors appear clean and safe. Director stated that they have outside cleaning vendor to clean the facility everyday in the after school hours. Furniture and equipment is age appropriate and in good condition free of sharp, loose, or pointed parts at the time of the inspection. Director stated that the school provided lunch and lunch was still prepared by the school in the school own kitchen as in the past, which is located in their after-school building at 19220 Stevens Creek Blvd, 2 blocks from the preschool facility. LPA observed Kitchen, food preparation, and storage areas appeared clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination, and Director states that contaminated food is discarded immediately. Trash cans for solid waste have tight-fitting covers on, and are in good repairs. Drinking water is readily available for the children in each room via water dispenser and in the outdoor playground area via water dispenser as well. Foods or beverages that are perishable are stored in covered containers at 45 (F) or less. Facility has 1 combined functioning smoke/carbon monoxide detector in each of classroom that meet statutory requirements.

Walk Through Outdoor: LPA observed no bodies of water outdoor. Playground equipment appears to be in good condition, free of sharp, loose, or pointed parts. Outdoor activity space appears to be in a safe condition and free of hazards. Playground has poured rubber foam for cushioning material under the climbing structures and slides.

Administration/Staff Records: Facility is operating within the conditions, limitations, and capacity specified on the license. Director understands that when CCL notify a licensee to immediately terminate the employment of, or to remove/bar any person with specified convictions or for other reasons, the licensee or Director shall comply with the notice. A review of staff records on today indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. LPA reminded Director of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the license and who come in contact with or provide care and supervision to the children. Penalty amounts: $100.00 per person per day, minimum of $100.00 to a maximum of $500.00 per person for an initial violation and a minimum of $100.00 to a maximum of $3000.00 per person for any subsequent violations within a 12-month period. Director understands upon notice of the Department to remove or to exclude an individual from the facility, Director shall immediately remove the individual and prevent them from returning to the facility or having contact with children in care.

LPA reviewed 3 staff records. Staff records contain appropriate documentation of education credits, Evaluation of Teacher Qualification, Health Screening Report, Personnel Record, Notice of Employee Rights, Criminal Record Statement, and Statement of Acknowledging Requirement to report Suspected Child Abuse. LPA observed at least one staff have current CPR & First Aid certification. LPA discussed SB 792 Immunization Requirement for staff with Director. LPA observed appropriate records for immunization against measles, pertussis, and influenza for Director and staff in file.

Child Records: LPA reviewed 8 children record. LPA observed child care facility roster, the sign in/out record reflects that all children are properly signed in/out by the person responsible for the child, Identification & Emergency Information, Admission Agreement, Physician’s Report, History Medical assessment, Consent for Medical Treatment are in each children file, Immunization records are maintained and up-to-date, Personal Rights and Notification of Parents’ Rights are in each child’s file.

Incidental Medical Services Incidental Medical Services (IMS) were discussed with Director. The facility is providing IMS at this time. LPA observed Epic Pen is current, and service consent is signed by For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Advisory: LPA discussed AB 2231: Civil Penalty Updates effective July 1, 2017 with Director. A $500 immediate civil penalty and $100 per day violation until corrected is assessed for serious violations such as absence of supervision, accessible bodies of water, accessible firearms, refused entry of licensing staff, presence of an excluded person, and violations that result in illness or injury. A $100 per day for each violation for Failure to Correct, and $250 immediately and $100 per day per violation until corrected for Repeat Violations. Licensing Forms, Title 22 Regulations, Resources & Information, Quarterly Updates, Videos can be obtained through the internet at http://www.ccld.ca.gov.

LPA reviewed COVID-19 infection prevention guidance & self-assessment with licensee including her plans in place to protect and support staff, children, cleaning & hygiene implementation, health screening & social distancing practices, meal times and arrival & pickup procedures. LPA observed posters & visitor policy notice were posted at the front entrance. LPA advised that all children above 2 were wearing masks.



LPA discussed Beginning January 1, 2018, Health and Safety Code 1596.8662 requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years except their primary language is not English can request to be exempted until Chinese version material becomes available. No deficiency was cited. Exit interview conducted with Director.
