25 | Licensing Program Analyst (LPA), Janet Tse, met with Jacqueline Alkhaseh, executive director to conduct an unannounced required annual inspection to the preschool today. LPA explained the nature of today’s visit to her. Present was also director Justin Fraser. LPA toured the indoor and outdoor of the facility during today’s visit. LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC 610), Earthquake Preparedness Checklist (LIC 9148), Parents’ Rights Poster (PUB 393), Personal Rights (LIC 613A), Child Car Seat Law (PUB 269), Menus (includes current and following week), and Activity Schedule. The Facility has an active waiver for electronic sign in/out. There are two buildings at the facility. Preschool operates from 6:30am to 2:30pm in Building 1, and from 6:30am to 6:00pm in Building 2.
A listing of staff criminal record clearances associated to this facility in the CCL Licensing Information System (LIS) on 05/08/2019 was reviewed; and it indicates that all Facility staff or other individuals who require caregiver background clearances have received criminal record and child abuse index clearances or exemptions.
LPA observed the medical assessment and the Information and Emergency Information form (LIC 700) in ten children’s files. LPA observed the required health screening in three staff files. Executive Director has current CPR and First Aid Certifications on file (expiration date: 03/05/2018). Executive Director understands that there shall be at least one person, with valid CPR and First Aid certifications, on site at all times or present during off-site activities (field trips). LPA observed that the teacher/child ratio was in compliance during today’s visit. Executive Director understands the conditions, limitations, and capacity specifications of the Facility license. Executive Director understands that children shall be visually supervised at all times. LPA reviewed the sign-in, sign-out sheets for the past 30 days and Child Care Facility Roster (LIC 9040) and all were in compliance. At arrival, LPA observed 38 napping children with two teachers in Building 2. There were no preschool children in Building 1.
LPA observed that all rooms are clean and safe for all children and staff. Director states that the Facility has |
