Licensing Program Analyst (LPA), Janet Tse, met with Jacqueline Alkhaseh, executive director to conduct an unannounced required annual inspection to the preschool today. LPA explained the nature of today’s visit to her. Present was also director Justin Fraser. LPA toured the indoor and outdoor of the facility during today’s visit. LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC 610), Earthquake Preparedness Checklist (LIC 9148), Parents’ Rights Poster (PUB 393), Personal Rights (LIC 613A), Child Car Seat Law (PUB 269), Menus (includes current and following week), and Activity Schedule. The Facility has an active waiver for electronic sign in/out. There are two buildings at the facility. Preschool operates from 6:30am to 2:30pm in Building 1, and from 6:30am to 6:00pm in Building 2.

A listing of staff criminal record clearances associated to this facility in the CCL Licensing Information System (LIS) on 05/08/2019 was reviewed; and it indicates that all Facility staff or other individuals who require caregiver background clearances have received criminal record and child abuse index clearances or exemptions.

LPA observed the medical assessment and the Information and Emergency Information form (LIC 700) in ten children’s files. LPA observed the required health screening in three staff files. Executive Director has current CPR and First Aid Certifications on file (expiration date: 03/05/2018). Executive Director understands that there shall be at least one person, with valid CPR and First Aid certifications, on site at all times or present during off-site activities (field trips). LPA observed that the teacher/child ratio was in compliance during today’s visit. Executive Director understands the conditions, limitations, and capacity specifications of the Facility license. Executive Director understands that children shall be visually supervised at all times. LPA reviewed the sign-in, sign-out sheets for the past 30 days and Child Care Facility Roster (LIC 9040) and all were in compliance. At arrival, LPA observed 38 napping children with two teachers in Building 2. There were no preschool children in Building 1.

a third-party cleaning service that cleans the Facility Monday through Friday in the evenings. Drinking water is readily available for the children in each room and in the outdoor playground area via water bottles, drinking fountains and water dispensers. LPA observed solid waste containers with tight-fitting lids in each room and in the playground areas. Staff and children’s bathrooms are clean, sanitary, and operable. There is a separate staff bathroom not utilized by the children which an isolated child can use if needed. Director states that there are no weapons on the premises.

The food preparation area is adequately equipped with an oven, a microwave oven, a refrigerators, and hot and cold running water. There is also adequate food for snacks for the children. Children bring their own lunch. Cleaning supplies and any poisons are stored inaccessible to the children. Any medications at the Facility are stored appropriately and in the director's office. LPA reviewed the medication lock box during today’s visit. Currently there is no children that needs medication and no current medicine log.

LPA observed all furniture and equipment is in good condition and safe for the children. The playground areas utilized by children is surrounded by appropriate fencing and the outdoor surfaces are safe for the children. LPA observed that the outdoor equipment is age appropriate and in good condition. There is also sufficient resilient materials (tan barks and sand) in the outdoor playground area. LPA did not observe any bodies of water.

LPA discussed Zero Tolerance with immediate $500 civil penalty; and an ongoing $100 per day per violation continues until the violation(s) is corrected. LPA discussed the requirements of AB633 with the director and provided her the AB633 fact sheet and a copy of Acknowledgement of Receipt of Licensing Reports (LIC 9224) and she understands the requirements. LPA observed the LIC 9224 for the conference on 10/15/2018 and the type A deficiency cited on 09/26/2018 with parents' signatures in each child's file. LPA reviewed three staff files and observed the required immunization records per SB792 and the Mandated Reporter AB1207 Compliant Child Care Training are in each staff's file.

Licensing Forms, Title 22 Regulations and Information can be obtained through the internet at http://www.ccld.ca.gov. LPA conducted an interview with the Executive Director and the Director prior to the conclusion of this inspection. The preschool is following the new supervision protocol, and continuous periodic training is provided to the staff. Director stated that classrooms and playgrounds are being spot check daily. A copy of the training outline and the plan of action for supervision were provided to LPA today.

No deficiency was cited. Notice of site visit was issued and must be posted for 30 days.
