NARRATIVE
1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	Licensing Program Analyst (LPAs), Marilou Monico and Pete Hernandez, met with Owner/Site Director, Nadia Bougachouch, and conducted a case management inspection in response to licensee's request to increase the capacity from 40 to 45 children. LPAs toured the indoor and outdoor areas. The following indoor measurements were taken on January 7, 2015 by LPA Monico: ROOM A5 15.332 x 17.581 = 269.551 minus 17.328 (encumbered space) = *252.223* ROOM A6 22.166 x 21.249 = 471.005 minus 105.985 (encumbered space) = 365.02 The following measurements were taken on July 27, 2016, by LPA Monico: ROOM A3/A4 35.667 x 15.500 = 552.838 minus 9.847 (encumbered space) = 542.991 The following measurements were taken by LPA Stephanie Rangel on July 6, 2018: ROOM A1/A2 15.083 x 32.333 = 487.679 minus 8 (encumbered space) = 479.679 *TOTAL PRESCHOOL INDOOR SPACE: 1,639.913 sq. ft. divided by 35 = 46 children.* There are 4 sinks (60), 4 toilets (60) and 2 urinals (30) available for the children. Hot and cold water is available in the children's sinks. Staff bathroom (women) and additional children's bathroom are located next to the Church's Office. The sick child will use the staff bathroom if needed and will be escorted by a staff member. LPAs observed trash cans with tight-fitting lids throughout the facility for the disposal of solid waste. There are 48 chairs, 50 cubbies, 9 tables, and 50 mats. LPA observed adequate toys, equipment and supplies for the children. Drinking water indoor is via individual water bottles. The following outdoor measurements were taken by LPA Rangel on January 29, 2018:
YARD 1 in front of A5 & A6 49.581 x 25.000 = 1,239.525 minus 91.332 (encumbered space) = 1,148.193

The following measurements were taken by LPA Rangel on July 6, 2018:

YARD 2 17.833 x 3.333 + 25.250 x 84.333 = 2,188.846

TOTAL OUTDOOR SPACE: 3,337.039 sq ft divided by 75 = 44 children

Yard 1 and 2 are enclosed with appropriate fencing. Shade is provided by trees and building overhang. Drinking water is readily available to children via individual water bottles.

Staff present during the inspection have cleared criminal record and child abuse index clearances or exemptions. LPAs also reminded Owner/Site Director of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the license and who come in contact with or provide care and supervision to the children. Penalty amounts: minimum of $100.00 to a maximum of $500.00 per person for an initial violation and a minimum of $100.00 to a maximum of $3000.00 per person for any subsequent violation within a 12 month period.

Fire Marshall Clearance was granted on December 5, 2019.

The following items must be completed prior to licensure of 45 preschool age children:
1) Revised facility sketch
2) Waiver request to be licensed for 45 children even the playgrounds are only measured for 44 children and outdoor schedules for Playground 1 & 2.
