25 | LPA Pete Hernandez met with Nadia Bougachouch, Director, for an unannounced Case Management - Incident inspection. The purpose of the inspection is to review an unusual incident report, UIR, that was self reported on 2/4/2022. This incident involved C1 having fell as the child was walking down stairs from of a climbing structure. This resulted in a cut near the child's right eye. The child was taken to a medical office for an evaluation that resulted in a Band-Aid. The climbing structure and the playground area was surrounded by the impact absorbent material which appears to be chipped wood. Child supervision is not at issue since the Teacher and Director were only a few feet away from the child when the incident occurred.
LPA inspected the physical plant. LPA interviewed staff. LPA also reviewed incident report, children's roster, and reviewed the child's file. The investigation by the LPA determined that the Teacher and Director acted in the child's best interest under the circumstances and there is no deficiency noted.
A deficiency is NOT being cited based on the LPA observation, interviews conducted, and record review in accordance with the California Code of Regulations, Title 22, An exit interview was conducted with the licensee. A copy of this report was discussed and left with the Licensee, Nadia Bougachouch, whose signature on this form confirm receipt of these documents.
A NOTICE OF SITE VISIT WAS ISSUED, POSTED NEAR THE ENTRANCE TO THE FACILITY, AND MUST REMAIN POSTED FOR 30 DAYS. |
