Licensing Program Analyst (LPA) Samantha Yip conducted an unannounced annual random inspection. LPA met with Lindsay Meazell, Assistant Director, and explained the reason for the inspection. Present during the inspection were 19 children and at least 5 staff present during the inspection. The staff child ratio was observed to be in compliance during today's inspection. Director Diana Manix arrived shortly after.

LPA reviewed the sign in/out sheet, which contains full signatures of authorized representative. License, Parent’s Rights, and Emergency Disaster Plan were observed to be posted.

LPA toured the inside and outside of the facility with Director and Assistant Director. The facility was observed to be clean. Disinfectants, cleaning solutions, and all other items that are dangerous to children were observed to be securely stored and inaccessible to children. The furniture and the equipment, such as tables, chair, napping cots, were age-appropriate and in good condition; free of sharp or loose, or pointed parts. The floors were clean and free of tripping hazard. LPA observed there is sufficient amount of age-appropriate toys for children in care. Restrooms for children’s use were observed to be sanitary, and in functioning condition. Storage containers for waste have tight fitted lids. Director stated that there are no weapons stored on the premise. LPA observed a fully charged fire extinguisher, a functioning carbon monoxide, and smoke detector. The last fire/disaster drill was last conducted on 07/2019.

Staff present during the inspection have cleared criminal record and child abuse index clearance or exemption. LPA also reminded Director of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the license and who come in contact with or provide care and supervision to the children. Penalty amounts: $100.00 per person per day, minimum of $100.00 to a maximum of $500.00 per person for an initial violation and a minimum of $100.00 to a maximum of $3000.00 per person for any subsequent violation within a 12 month period.

Director stated that she currently does not have any children in care who requires Incidental Medical Services (IMS) services. IMS policy was discussed.

As a result of today's inspection, two Type B deficiencies have been cited. An exit interview was conducted, where this report, the citation, plan of correction, and appeal rights were discussed with Director.

The outdoor activity area is fenced. There is resilient materials to absorbs falls. LPA reminded Director to check there is sufficient amount of resilient materials to absorb falls. LPA also observed that there are rubber mats at the end of the slides and the steps. Rubber mats are stacked on top of each other and were uneven. LPA explained to Director how this could pose a risk to the children in care if they were to step on the uneven section. Director stated that will replace it with one large rubber mat. She will have staff sweep the outdoor area to before children's use to ensure rubber mats are stacked evenly. LPA also observed that one of the play equipment and shed outside is cracked. Assistant Director placed tape over the crack area. Shaded rest areas are provided through building overhang. LPA did not observe any bodies of water. Lunch is prepared and brought from home. The facility does provides breakfast and snacks to the children in care. Menu was posted with serving size. Kitchen used to prepare breakfast and snacks was observed to be clean and sanitary. Drinking water is provided through water bottles and drinking fountains.

Facility roster was reviewed and was copy was obtained. 10 children’s files were reviewed during today’s inspection. The records reviewed include not limited admission agreement, immunization records, and parent's rights.

5 staff files were reviewed during inspection. The records reviewed include but not limited to immunization (Measles, Pertussis, and Influenza), Health Screening with TB test results, Criminal Record Statement, and Mandated Reporter Training (AB 1207). There is currently at least one staff on site with valid CPR/First Aid, which expires 04/14/2021.
LPA discussed Assembly Bill (AB) 1207 (Mandated Child Abuse Reporting Training) which is required training that began on January 1, 2018 and requires renewal every two years. Mandated Reported Training can be accessed at http://www.mandatedreporterca.com.
