Licensing Program Analysts (LPAs) Marilou Monico and Jessica Bongardt conducted a Case Management Inspection in response to licensee's request to add two classrooms on the license. LPAs met with Facility Representatives, Madeline Fraser (Admin Preschool), Amelia Lalley (Clinical Manager), and Maureen McNeil (Faces Administrative Manager), and the purpose of the inspection was explained. The preschool program is licensed to serve ages 2.5 years to 6 years old, for 20 children. The preschool program is located at Cory Elementary School campus. The preschool program operates Monday through Friday from 9:00 AM to 12:00 PM. LPAs toured the facility both inside and out, as well as measured the indoor space during today's visit. The San Jose Fire Department granted the fire clearance on August 3, 2023.

Indoor Activity Space:
PRESCHOOL 1: 25.667 x 31.500 = 808.510 minus 94.399 (encumbered space) = 714.111
ROOM 33: 18.583 x 23.250 = 432.054 minus 25.057 (encumbered space) = 406.997
PRESCHOOL 2: 31.500 x 35.667 = 1,123.510 minus 89.389 (encumbered space)= 1,034.121
INDOOR SENSORY GYM: 39.000 x 24.833 = 968.487 minus 141.524 (encumbered space) = 826.963
TOTAL INDOOR SPACE = 2,982.192 sq. ft. divided by 35 = 85 children

The facility has 5 sinks (75) and 6 toilets (90) available for the children. The sinks used by children only have cold running water. There are two staff bathrooms adjacent to the Main Office. The isolation of sick children will be in the Main Office and isolation equipment will be provided. Sick children will use one of staff bathrooms next to the Material Closet if needed and will be accompanied by a staff member. There are 32 chairs, 10 tables, and 28 cubbies for the children.

Exit interview was conducted and report was reviewed with Facility Representative, Madeline Fraser, and advised her that the additional classrooms, Preschool 2 and Indoor Sensory Gym, are approved for daycare use.

There were no deficiencies cited.
