Licensing Program Analyst (LPA) Shine Yu conducted an unannounced complaint investigation to deliver the finding. LPA met with teacher, Yaping Qiao. Present during investigation were 3 staff and 20 preschool children.

It was alleged that staff do not keep the classrooms and shared area clean.

On 02/03/26, LPA toured the facility, observed the 3 classrooms and conducted interviews with the director and teachers present during inspection. LPA obtained the children's roster. Based on the observations and interviews, all teachers who were interviewed stated they would lead students to clean up classrooms at the end of each day. There’s a dedicated teacher responsible for classrooms vacuuming daily and cleaning up after meal time in the shared area. LPA observed all 3 classrooms and shared area were clean and in working condition during the inspection.

On 03/04/26, LPA called 7 parents. All parents who were interviewed stated they did not observe the classrooms and shared area unclean. LPA also contacted Reporting Party by email and received no further information.

Based on LPA’s observation and interviews, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegations are UNSUBSTANTIATED.

Exit interview conducted and copy of this report was reviewed in Mandarin with the teacher, Yaping Qiao. A copy of Appeal Rights was given.

Licensing Program Analyst (LPA) Shine Yu conducted an unannounced complaint investigation to deliver the finding. LPA met with teacher, Yaping Qiao. Present during investigation were 3 staff and 20 preschool children.

It was alleged that not enough teachers present to supervise children at nap time.

On 02/03/26, LPA toured the facility, observed all 3 classrooms and conducted interviews with the director and teachers present during inspection. LPA obtained the children's roster. Based on the interviews, if a child needs to use restroom during the nap time, the supervising teacher won’t be able to observe the child after child entering to the restroom. The director stated this scenario although rarely happened, but happened few times in a year, since the teacher would always ask children to use restroom before the nap time. LPA reviewed the sketch floor plan and observed the classrooms and bathroom layout, when teacher stands by the classroom (Peace) door, there will be no supervision after child enters into the restroom.

On 03/04/26, LPA called 7 parents. All parents who were interviewed stated they did not observe teacher / children ratio and lack of supervision issues.

Based on LPA’s interviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.

As a result, Type A deficiency was cited on the following page:

LPA informed the teacher, Yaping Qiao, that this report dated 03/09/2026 document one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.

Also, LPA informed the teacher to provide a copy of this licensing report dated 03/09/2026 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
