25 | Licensing Program Analyst (LPA), Stephanie Collins and Licensing Program Manager (LPM) Gladys Kuizon, met with Lindsay Masuda preschool director, for an unannounced annual inspection. The Infant/Toddler programs are licensed in Rooms 165 and 166. The facility also has a licensed Preschool program (#434406751)on the same campus. Hours of operation: 7 AM to 6:00 PM.
There were 8 children in room # 165 with (4) four staff supervising and 7 children in room # 166 and 2 staff supervising a total of 15 children and (6) six staff.
LPA toured all rooms and outdoor space of the facility during today's inspection. LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC 610), Earthquake Preparedness Checklist (LIC 9148), Parents' Rights Poster (PUB 393), Personal Rights (LIC 613A), Child Car Seat Law (PUB 269), Menus, and Activity Schedule.
LPA reviewed 10 percent of the children's and staff files during today's inspection. Each child's file reviewed contains the Information and Emergency Information form (LIC 700) and current immunization records. Staff files reviewed contain the required transcripts/verification of experience/proof of current enrollment (when applicable) and required vaccination records. All preschool staff have completed the required Mandated Reporter Training for Child Care Workers and Preventive Health & Safety Training for Director. There is at least one staff in each room with valid CPR & First Aid certifications on file. Lindsay Masuda understands that there shall be at least one person, with valid CPR and First Aid certifications, on site at all times or present during off-site activities (field trips).
LPA observed that the teacher/child ratio within the facility was in compliance during today's inspection. All preschool children in attendance were signed in on the electronic sign in/sign out system utilized by the Facility. Lindsay Masuda understands the conditions, limitations, and capacity specifications of the facility license. Lindsay Masuda understands that all children shall be visually supervised at all times and never left unattended.
