25 | Licensing Program Analysts (LPAs) Kassandra Medrano and Mel Matos met with co-director Doug Ikemoto for a case management inspection. LPA's explained the nature of today's inspection to Doug.The Facility is currently closed for summer break and will reopen on 8/17/2022. The program is currently licensed in Rooms 5,6,7,8, and Library.The program has requested to add two classrooms (Rooms F & 24) without changing their capacity. Measurements of Rooms F & 24 were taken during today's inspection.
Since the program will be operated at a functioning elementary school site, the facility is exempt from the indoor and outdoor square footage requirements, fencing requirements, and toilet/sink ratios and isolation area requirements. The Facility has yet to submit an updated capacity letter and lease/rental agreement from the San Jose Unified School Districts that reflects the new additions, rooms F and 24.The following measurements were taken today:
INDOOR MEASUREMENTS:
Room F: (14 x 8=118) + (31.167 x 23.167=722.05) = 1,693.06
(14 x 8=118)+(31.167 x 23.583=735.01)
Room 24: 39 x 21.750= 848.25
TOTAL INDOOR SPACE: 2,541.31 sq. ft. divided by 35 = 72 children
During the inspection it was observed that the two rooms requested for use by the Facility are still licensed by YMCA @ Booksin (430709472). The Facility has exclusive use of the kindergarten playground. LPAs observed adequate shade (building overhang and trees in the Kindergarten playground. The play structure in the Kindergarten playground is designated for ages 5-12 year olds. The structure has artificial turf and cushioning underneath as resilient materials. LPA's discussed the usage of the In this yard, LPAs observed appropriate/sufficient indoor/outdoor materials/supplies for the preschool children.
LPAs conducted an exit interview with Doug Ikemoto, co-director, and advised him that Rooms F & 24 will be approved & added to the Licensee upon receipt of the following:
1. Fire Clearance approval from the City of San Jose Fire Department.
2. Rental/lease agreement from the San Jose Unified School District verifying Control of Property for existing licensed rooms: Rooms 5, 6, 7, 8, & Library plus additional Rooms F & 24.
3. Capacity letter from the San Jose Unified School District for all rooms.
4. YMCA @ Booksin (430709472) has submitted a request to the Department requesting to drop Rooms F and 24 from their existing license.
No deficiencies issued and Notice of site visit issued and must remain posted for 30 days. |
