25 | Licensing Program Analyst (LPA) Mel Matos met with Giovanna Sivira Gallardo, Licensee, for an unannounced Required – 1 year annual inspection. LPA was granted access to the home by the Licensee. LPA also observed two adult assistants and eleven day care children (2 infants & 9 preschool) in the home during today's inspection. Licensee was operating within her capacity and ratio requirements. LPA observed the required postings, including the facility license, near the front entrance to the day care. Days and hours of operation are Monday - Friday from 7:30 AM to 5:30 PM. The adults residing in the home are: Licensee and Licensee's spouse (Andres Serva). Licensee has one 17 year old son residing in the home.
LPA reviewed a current Child Care Facility Roster and Fire/Disaster drill log during today's inspection. The last fire/disaster drill was completed on March 3, 2023. Licensee states she has insurance for the day care through DCI insurance. Licensee has current CPR and First Aid certifications (expiration: 03/04/2025). Licensee & two adult assistants has the required vaccines (MMR, Tdap, & flu) and is current with the Mandated Reporter Training for Child Care Workers. LPA reviewed six children's files and the files were complete with the required forms. LPA reviewed three staff files (Licensee & two adult assistants) and the files were complete with the required forms/documents. Licensee states that a child will be isolated in the living room area if necessary due to illness or communicable disease.
LPA toured the indoor and outdoor areas of the home during today's inspection. Licensee has a working telephone in the home. The home is clean, orderly, (including central heating/ventilation), and safe for the day care children. There are safe and age appropriate toys, play equipment, and materials for the children in the home. There are no wall heater units in the home. The home is two story and LPA observed barricaded stairs during today's inspection. Off limit areas inside the home (upstairs): 3 bedrooms and 2 bathrooms. Off limit areas inside the home (downstairs): attached garage, laundry room, kitchen, storage closet, and barricaded fireplace unit. Off limit area outdoors: locked storage shed.
LPA observed a fully charged 2A10BC fire extinguisher, working smoke/carbon monoxide detectors, no bodies of water, and fenced backyard. Licensee's spouse has 5 rifles, 7 handguns, and ammunition in the home. All items are stored in the attached garage area in a secure Winchester gun safe. All weapons are not loaded and ammunition is stored in a separate lock box in the gun safe. Licensee states that she has two Frenchies & one American bulldog (licensed & vaccinated) that she keeps in off limits areas during day care hours. |
