1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	Licensing Program Analyst (LPA) Mel Matos conducted an announced tele-prelicensing inspection via FaceTime (video call) with Shanam Saini, Applicant representative/director, and Sahil Saini, Shanam's board officer/Shanam's brother. Purpose of today's tele-prelicensing inspection: change of ownership (corporate status) and name change of the Facility. The existing facility is under the corporate & facility name - Windsor Academy, LLC (facility #434412911). LPA advised Shanam that due to the COVID-19 pandemic and "shelter in place" order, today's report will be emailed to her (email: shanamsaini@gmail.com) and Shanam's reply to the email will serve as acknowledgement that Shanam received/understood today's report. The indoor and outdoor areas of the Facility were observed by LPA via FaceTime (video call). The Facility has five classrooms indoors (Classrooms 1, 2, 3, & 4, Activities Area). There is one playground outdoors. The measurements of the indoor and outdoor areas were taken by Shanam Saini and Sahil Saini as LPA observed the measuring process and provided directions via FaceTime (video call). Indoor measurements: Classroom 1: 18.000 x 14.000 = 252.000 Classroom 2: 15.000 x 17.750 = 266.250 Classroom 3: 15.667 x 13.250 = 207.587 Classroom 4: 10.250 x 10.833 = 111.038 Activities Area: 22.917 x 11.750 = 269.274 Total indoor space = 1,106.149 sq ft divided by 35 = 31 preschool children.

OUTDOOR MEASUREMENTS:

Playground: (65.000 x 39.000 = 2,535.000) minus 238.667 encumbered space = 2,296.333

Total outdoor space = 2,296.333 sq ft divided by 75 =30 preschool children.

LPA observed that there is adequate fencing surrounding the playground. The outdoor activity space is equipped with a variety of age-appropriate toys and equipment such as a play structures (which are secure and safe for the children). Areas around and under the play structure have artificial grass and cushioning material underneath for resilient materials. Shade is provided by building overhangs and trees. LPA did not observe any bodies of water during today's inspection. Transportation will not be provided by the Facility.

There are 2 toilets (30) and 2 sinks (30) inside the facility for children to use. There is one bathroom with one toilet and one sink in near Classroom 4 for staff. Children who become ill will use the staff restroom and will be isolated in the director's office with isolation equipment. LPA observed soap dispensers and disposable paper towels in the children and staff bathrooms. The sinks used by children only have cold running water.

LPA did not observe any wall heaters within the Facility. The Facility does have a barricaded fireplace unit in Classroom 2. The Facility has central heating/air conditioning in all classrooms. All preschool classrooms have appropriate lighting. First aid supplies are stored in each Classroom inaccessible to the children. Shanam states that the Facility does not administer any medications.

The food preparation area (kitchen) is functional and has hot and cold running water, sink, refrigerator, microwave, oven, and storage space for food. The Facility only provides AM/PM snacks for the children. Shanam states that parents are responsible for providing lunch for the children. Cleaning and sanitation of dishes and utensils will be done via dishwasher. Disposable utensils and plates will be utilized by the Facility. LPA observed a solid waste container with a tight-fitting lid in each Classroom. Cleaning supplies will be stored on high shelves in each Classroom inaccessible to children. Shanam states that the Facility has adequate cleaning/sanitizing supplies & PPE equipment.

There are 32 chairs, 6 tables, 36 cubbies, and 30 mats for the children. There is adequate equipment, supplies and toys for the preschool children in each Classroom. Drinking water is readily available for the children in the Facility and in the outdoor playground area via water bottles & pitchers/disposable cups. There is a working telephone at the Facility. The phone number is 408-746-5666.

Shanam Saini, Applicant representative/director has the required criminal record clearances and Child Abuse Index Checks. She also meets the education and experience qualification requirement and has completed 16 hours of training on preventive health practices, which includes pediatric cardiopulmonary resuscitation and pediatric first aid, and one hour of Child Care Nutrition. Shanam's CPR/1st Aid Certifications expire 04/28/2021. Shanam has a current Child Development Site Supervisor Permit issued by the CA Commission on Teacher Credentialing that expires on 04/01/2022. Shanam's Mandated Reporter Training for Child Care Provider Certification expires 06/15/2022.

LPA reminded Shanam of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the facility, and who come in contact with or provide care and supervision to the children. For an initial violation, civil penalty amounts to $100.00 per person per day up to $500.00 per person. For a subsequent violation within a 12 month period, civil penalty amounts to $100.00 per person up to $3000.00 per person.

A fire clearance approval for 30 preschool children was received from the Sunnyvale Department of Public Safety on July 8, 2020. Santa Clara County Department of Public Health (sccgov.org) website for additional information/updates re: COVID-19.

An exit interview was conducted prior to the conclusion of today's tele-prelicensing inspection via FaceTime (video call). Shanam was advised that a license for 30 preschool children will be approved upon receipt of the following:
