25 | On 06/07/2023 at 8:30 AM, Licensing Program Analyst (LPA) Susy Cervantes, met with assistant Barbara Santoyo, for a case management visit. Present was assistant and licensee's spouse with 10 children in care: six infants and four preschool. Licensee arrived shortly after and two additional preschool children arrived during visit.
LPA informed Licensee, Maria De Jesus Santoya-Morales, and assistant that there are too many infants in care, LPA reminded staff that there can only be four infants in care when there are no school age children. LPA reviewed assistants CPR and first aid as well as their mandated reporter training, assistant's CPR and first aid has expired (11/2022). LPA was informed assistant has a class scheduled. LPA asked if spouse has pediatric CPR and first aid, LPA was informed that they did, but was not able to verify.
Type A deficiencies were cited during today's visit. LPA Susy Cervantes informed licensee, Maria de Jesus Santoyo-Morales, that this report dated 06/07/2023 documents two Type A citations which shall be posted for 30 consecutive days as there are immediate risks to the health, safety, or personal rights of children in care.
Also, LPA Cervantes informed the licensee, Maria De Jesus Santoyo-Morales, to provide a copy of this licensing report dated 06/07/2023 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview conducted and report was reviewed with the licensee, Maria de Jesus Santoyo-Morales. Licensee was informed that failure to correct the deficiencies by the specified Plan of Correction Due Date will result in assessment of civil penalties in the amount of $100 per day per violation until the correction is made. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
