13 | On 10/13/2025, Licensing Program Analyst (LPA) Jaime Gonzaleswas greeted and granted access to the facility by a staff member. Staff member stated that Director Hui Shen was out today, and Assistant Director Qun Dai was out on vacation. LPA met with Regional Director Connie Lomas-Fischer and stated the purpose for today’s visit was to deliver the findings for the complaint investigation for the allegation listed above. LPA toured the facility and completed a census count. Present in Room 1 were 10 preschoolers and 2 staff. In Room 2 were 13 preschoolers and 3 staff. In Room 3 were 12 preschoolers and 2 staff. In Room 4 were 11 preschoolers and 2 staff. In Room 5 were13 preschoolers and 2 staff. In Room 6 were 8 preschoolers and 2 staff. During the investigation Site Director, Hui Shen arrived at the facility.
During the course of the investigation, LPA collected relevant documentation, conducted interviews with the Regional Director, Director, Assistant Director, children and parents. Based on the available evidence, it is concluded that although the allegation listed above violation that Staff (S1) physically abused day care child may have happened or is valid, there is not a preponderance of evidence to prove the alleged did or did not occur. Therefore, the allegation is thus UNSUBSTANTIATED.
As a result of this investigation, no deficiencies were cited. Exit interview was conducted and report was reviewed with Site Director, Hui Shen and Regional Director, Connie Lomas-Fischer.
A NOTICE OF SITE VISIT WAS GIVEN AND MUST REMAIN POSTED FOR 30 DAYS. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
