1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	Licensing Program Analyst (LPAs) Ashley Lopez and Kassandra Medrano conducted an unannounced Annual Required Inspection. LPAs met with Hui Shen, who guided LPAs on a tour of the facility indoors and outdoors.The school consists of 6 of classrooms. This program operates from 8:30am - 6pm Monday through Friday. This facility has a capacity of 111 children; preschool children ages 2 to first grade. Teacher-child ratio was met during the visit. All individuals present have obtained a criminal record clearance. All children were observed to be always under visual supervision of a teacher. LPA observed the facility to be clean, safe, sanitary and in good repair. Furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. Indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and hand washing facilities are in safe and sanitary operating conditions. The administrators building is used as an isolation area. Parents are contacted immediately when children are feeling ill. Food menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. Some preschoolers bring their lunch from home and the facility may also provide the children with Breakfast, Lunch, and PM Snack. All food is protected from contamination, and LPA inquired about any contaminated food and that it is discarded immediately. Children bring their water bottles from home and water is provided via filtered water faucets if needed. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in a locked area that is inaccessible to children. ----------------------------------continued on page 2-------------------------------------------

Facility has functioning smoke detector/carbon monoxide detector that meets statutory requirements. Outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. Outdoor activity space surface is maintained in a safe condition as is free of hazards. The director states that there are no bodies of water on the premises and LPA did not observe any bodies of water during this visit. Areas around around and under climbing equipment have soft cushioning to absorb a fall.

Facility uses an electronic sign in/out program called Pro Care. Parents sign in/out children at the facility through an IPad or their phone app. LPA reviewed 10 children's files and 10 staff files during today's inspection. Children’s Records were reviewed for completeness; Inspection of required forms was made. In review of children’s records, files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary. All staff files reviewed contain the required transcripts, immunization records, and Health Screening Report. All staff have current certificates of completion of the Mandated Reporter Training for Child Care Workers on file. At least one staff member has current CPR and First Aid certifications on file. Director understands that there shall be at least one person, with valid CPR and First Aid certifications, on site at all times or present during off-site activities.

SB792 (Immunization Requirements for Staff and Employees) was discussed with the facility representative. AB1207 Mandated Child Abuse Reporting – On or before March 30, 2018 any person who works in a child care facility shall complete the training and renew the training every 2 years. Incidental Medical Services (IMS) policy was discussed. This facility will provide licensing with an up to date Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information, see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
