Licensing Program Analyst (LPA) Stephanie Collins Acting Site Supervisor Angie Saechao , for an unannounced annual/random inspection. LPA toured the Facility both the indoor and outdoor areas of the Facility during today's inspection. LPA notes that the Facility is licensed for room four were three infants were in care with 3 staff members assisting and interacting with them. Licensee holds a license #434413688 to operate a Preschool Center next door at 1682 Berryessa Rd. San Jose CA. 95133.
LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC 610), Earthquake Preparedness Checklist (LIC 9148), Parents' Rights Poster (PUB 393), Personal Rights (LIC 613A), Child Car Seat Law (PUB 269), and Activity Schedule.

A review of staff records on December 12 , 2019 indicates that all Facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. LPA reminded Angie Saechao of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the license and who come in contact with or provide care and supervision to the children. Penalty amounts: $100.00 per person per day, minimum of $100.00 to a maximum of $500.00 per person for an initial violation and a minimum of $100.00 to a maximum of $3000.00 per person for any subsequent violations within a 12-month period.

LPA reviewed three children's and staff files during today's inspection. Each child's file reviewed contains required Information and Emergency Information forms (LIC 700) and Needs & Services Plans. LPA reminded Angie the Needs & Services Plans also includes individual feeding plans for each child and are updated at least once per quarter. All staff files reviewed contain the required transcripts/verification of experience. All infant teachers are qualified or in process of completing the infant/toddler course. The Facility has at least one staff in the room with current CPR and First Aid certifications on file. Angie understands that there shall be at least one person, with valid CPR and First Aid certifications..

LPA observed that the teacher/child ratio was in compliance during today's inspection. LPA observed 3 infants with 1 teacher and 2 aides in Infant Room 4, Angie understands that infants shall be visually supervised at all times and never left unattended, including in the crib/nap areas.

LPA observed that the room is clean and safe for all children and staff. The facility has one infant changing table within arm’s in reach of a sink. The sleeping area for infants is physically separated from the indoor activity space by clear partitions. Drinking water is readily available for the children in the room and in the outdoor playground area via water bottles and sippie cups. First aid supplies are stored under the changing table and inaccessible to infants. LPA observed solid waste containers with tight-fitting lids in the room. Staff and children's bathrooms are clean, sanitary, and operable. There is a separate staff bathroom not utilized by the children which an isolated child can use if needed. Angie states that there are no weapons or firearms on the premises. Fire and carbon monoxide detectors and fully charged fire extinguishers were observed.

The food preparation and storage areas are clean, free of litter & rubbish, and free of rodents and other vermin. All food and beverages that require refrigeration are stored in covered containers at 45 degrees F or less. Cleaning supplies are inaccessible to the children and stored in high cabinets/locked cabinets inaccessible to children.

LPA observed all furniture and equipment is in good condition and safe for the children. The playground area utilized by the infant children is surrounded by appropriate fencing and the outdoor surfaces are safe for the children. The infant playground is shared with the preschool. A waver has been granted and is posted with schedule in class room. LPA observed that the outdoor play equipment is age appropriate and in good condition. There are sufficient resilient materials in the outdoor playground area. LPA did not observe any bodies of water.

LPA conducted an exit interview with Acting Site Supervisor Angie Saechao and advised her of the pending Department regulation update Re: safe sleep for infant children & the new requirement requiring all facilities test water for lead as of January 1, 2020. LPA referred Angie to the Department website: www.ccld.ca.gov for additional information.

No deficiencies issued during today's inspection.

A NOTICE OF SITE VISIT WAS ISSUED, POSTED NEAR THE ENTRANCE TO THE FACILITY, AND MUST REMAIN POSTED FOR 30 DAYS.
