25 | Licensing Program Analyst (LPA) Janette Cruz met with Khanh Than, Licensee, for an
unannounced Required – 1 year annual inspection. LPA explained the purpose of the visit and was granted access to the home by the Licensee. LPA also observed 12 children (2 infants and 10 preschool), Licensee's mother and Licensee's adult assistant in the home during today's inspection. The Licensee was operating within her capacity and ratio requirements during today's inspection. LPA observed the required postings, including the facility license, located in the living room of the home. The home has a working telephone (408) 597-7816. Days and hours of operation are Monday - Friday from 7:00AM to 6:00PM. The Licensee, her spouse, Vu Le and Licensee's renters, Graciela Ariasmunoz, Claudia Diazgomez, Talita Miranda and Esther McKoon are the adults residing in the home. The Licensee's CPR and First Aid on file expires on 04/2023.
LPA reviewed a current Child Care Facility Roster and Fire/Disaster drill log during today's inspection. The last fire/disaster drill was completed on 07/2021. The Licensee and adult assistants have the required vaccinations (MMR, Tdap, & flu opt-out). The Licensee’s Mandated Reporter Training for Child Care Workers on file expired on 08/10/21. LPA reviewed 12 children's files and observed current and updated Identification and Emergency Information (LIC 700) and the Family Child Care Home Notification of Parents' Rights forms (LIC 995A) in each file. The Licensee states that a child will be isolated in the living room if needed to because of illness or communicable disease.
LPA discussed the new “Safe Sleep” regulations with the Licensee and provided a copy of the regulations, including the Individual Infant Sleeping Plan (LIC 9227) form to the Licensee.
