Licensing Program Analyst (LPA) Stephanie Collins conducted a scheduled Tele-Pre-Licensing inspection via video conference call. Due to the COVID-19 situation and "Shelter in Place" Order, this LIC809 Facility Evaluation Report was generated at the Licensing Office and will be emailed to Applicant. Applicant's reply to the email will serve as acknowledgement that the report was received.

LPA met with Administer Vamdevan Namboodiri and Center Director Sujatha Namboodiri. Applicant VIDIARAMBH CUPERTINO LLC is applying for a License to operate an Infant Center with a capacity of 12 infants at the above listed address. The Infant Center is located in Rooms: 4. The Center will operate Monday through Friday from 8:00 AM- 6:30 PM, serving children ages 12 month until 24 months of age. Facility's phone # (408) 444-5019.
LPA assisted Director and Administer during Tele-inspection via Smartphone. The building was inspected inside and out.
Indoor space for the Infant Center was measured to ensure that there is 35 square feet of indoor activity space per child based on the total licensed capacity.

Indoor Activity Space Measurements are as follows:

Room 4 - INFANTS
= (Total sq. ft.) – (Encumbered Space) = (444.48 sq. ft.) – (10.88 sq. ft.)
= 433.6 sq. ft.

Total Indoor Activity Space = (433.60 sq. ft.) divided by (35 sq. ft. / child) = 12 Children

Outdoor space was measured to ensure that there is at least 75 square feet of outdoor activity space per child based on the total licensed capacity.

Outdoor Activity Space Measurements are as follows:
INFANT YARD
= (Total sq. ft.) – (Encumbered Space)
= (643.73 sq. ft.) – (0.00 sq. ft.)
= 643.73 sq. ft.

Total Outdoor Activity Space = (643.73 sq. ft.) divided by (75 sq. ft. / child) = 8 children

LPA observed age appropriate outdoor play equipment for the infant and there is resilient materials in play area to absorb falls. The outdoor play space is surrounded by fencing. Shade is provided by overhang and portable outdoor canopy will be used as needed. LPA did not observe any bodies of water during the visit. Director stated that facility will not provide transportation.

Facility has sufficient age appropriate toys, furniture, and equipment. LPA observed feeding chairs, cushioned floor mate for activities, hooks, and cubbies. No cribs were observed on the grounds. The infant center will only be licensed to service older infants 12 months to 24 months who are considered able to climb out of their cribs. Therefore, napping cots will be utilized during rest or nap time. Each mat or cot shall be occupied by only one infant at a time. Cots shall be arranged to provide a walkway between cots/ mates to allow staff to reach each infants without having to step over or reach over any other infant. Cots/Mats shall be equipped with a sheet to cover the cot or mat: depending on the weather a sheet and /or blanket to cover the child.

Director is aware NO infants under 12 months of age can be served under the pending/current license due to no cribs being on site and nap space not physical separated from the indoor activity space.

The infant room has two sinks and a changing table within arm's reach of a sink. The changing table has raised sides that are at least 3 inches high and a padded surface that is no less than one inch thick and covered in washable vinyl. The Director understands that a baby walker shall not be kept or used on the premises of a child day care facility. Drinking water is readily available indoor and outdoor. The Infant Center will be providing potty training. Facility will follow the states required Needs and Service Plan for each infant which is required to be updated every 3 months as well as a Feeding Plan. Appropriate seating equipment will be used to seat infants in during feedings. Infants shall be held during feedings if they are unable to sit unassisted. Propped bottles are not permitted. The lunch, morning and Afternoon snacks Menu will be posted. Food can be stored and prepared in the Kitchen. There is a sink with hot/cold running water, refrigerator and solid waste containers with tight-fitting lids. Areas designated for storage of detergents, cleaning compounds, sharps, medicines, and other items which could pose a danger if readily available to children are inaccessible to infants. the center has one or more carbon monoxide detectors that meets the established standards, A fully charged fire extinguisher was also observed. The center is equipped to isolate and care for any child who becomes ill during the day. The directors office will serve as the isolation area for any sick child. There is a separate staff restroom in the commend area sick children can utilize if needed.
LPA observed first-aid supplies located in a location accessible to staff but inaccessible to children
Director Sujatha Namboodiri meets Educational Qualification requirement. There is at least one staff with current certification for preventive health practices training with Lead poisoning, which included pediatric cardiopulmonary resuscitation and pediatric first aid certification. Director maintains documentation of immunity against Measles and Pertussis and required training. Directors' Background Check Clearance has been transferred to the Infant Center.
Director and Administer are reminded of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the license, and who come in contact with or provide care and supervision to the children. For an initial violation, civil penalty amounts to $100.00 per person up to $500.00 per person. For a subsequent violation within a 12-month period, civil penalty amounts to $100.00 per person up to $3000.00 per person.
