Licensing Program Analyst (LPA), Oscar Huang, conducted an announced prelicensing inspection to the Facility today. The Applicant has applied for a combination center (preschool #: 434416419, school-age #: 434416420 and infant #: 434416421) on the same campus. The Facility is located on the district office campus of the Cambrian Unified School District. LPA met with Manali Doshi, Applicant representative and explained the nature of today's visit to her. The facility is currently operated under Early Discoveries Child Development. Current owner is moving out of state and selling it to applicant. The infant program will be using Room 3 of Building 2, the preschool program will be using Rooms 11 & 12 of Building 3 and Room 17 of Building 4, and the school-age program will be using Rooms 13, 14, 15, 16 & music room of Building 4. LPA toured and measured the indoor and outdoor areas of the Facility during today's inspection. The School-Age Center will operate Monday through Friday from 07:00 AM to 06:30 PM, serving children ages Five years old until Eighth Grade. Facility's phone # 408-559-1003.

Indoor space for the School-Age Center was measured to ensure that there is 35 square feet of indoor activity space per child based on the total licensed capacity.

Room 13 of Building 4 (After School)

Outdoor space was measured to ensure that there is at least 75 square feet of outdoor activity space per child based on the total licensed capacity.

Outdoor Activity Space Measurements are as follows:
Yard 1 (Front of Building 4)
= (Total sq. ft.) – (Encumbered Space) = ( 9691.461 sq. ft.) – (0 sq. ft.)= 9691.461 sq. ft.

Total Outdoor Activity Space = (9691.461 sq. ft.) divided by (75 sq. ft. / child) = 129 children.

LPA observed age appropriate outdoor play equipment for the school-age children and the play field is covered by grass. Shade is provided by building overhang and trees. LPA did not observe any bodies of water during the visit. Applicant stated that facility will not provide transportation. Drinking water is readily available indoor and outdoor via tap & pitcher. Applicant stated that the facility will be providing transportation via school vans.

There are a total of 4 toilets (60) and 7 sinks (105) for the school-age children to use. LPA observed soap dispensers and disposable towels in the restrooms. The two restrooms near the Director's Office are designated for staff and children who become ill to use. The Director's Office will serve as an isolation area for children who are ill and has equipment (cots, chairs etc.) for children to rest .and the child will be supervised by staff during isolation. LPA observed that Facility has toys, furniture, material, and equipment for the children to use. The rooms are furnished with enough tables, chairs, and cubbies and hooks. There will be no napping for school-age children. There were no fireplaces or open space heaters observed. Room has lighting and ventilation for safety and comfort of school-age in care. First aid supplies and Medications were observed in each classroom inaccessible to children.

Facility will provide Meals and Snacks for the school-age children. Meals and Snacks will be stored and prepared at the facility center kitchen in building 4 near the Director's office. LPA observed the Kitchen was equipped with refrigerator, microwave, oven, and stove to store, heat, and/or cool the food as needed. Menu was posted. Containers used for storage of solid wastes have tight-fitting cover on. Detergents, cleaning compounds, sharps, medicines, and other items which could pose a danger if readily available to children are stored where they are inaccessible to children. Storage/Maintenance rooms were locked. Facility has janitorial service at the end of each day.

Director Kanika Sanchez meets Director’s Educational Qualification requirement. There is at least one staff with current certification for preventive health practices training, which included current pediatric cardiopulmonary resuscitation and pediatric first aid certification. Director maintains documentation of immunity against Measles and Pertussis and required training. Director and applicant's Background Check Clearance has been transferred to the school-age Center. LPA received all other employee criminal background clearance transfer requests on site.

LPA reminded Applicant of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the license, and who come in contact with or provide care and supervision to the children. For an initial violation, civil penalty amounts to $100.00 per person up to $500.00 per person. For a subsequent violation within a 12-month period, civil penalty amounts to $100.00 per person up to $3000.00 per person.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual – Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS has been submitted to the Department. The department has received a fire inspection approval from the San Jose Fire Department on 12/24/2019.

LPA received an updated application on site during today's inspection.

Exit interview was conducted, where this report was reviewed and discussed with Applicant and informed her that a license for 52 school-age children is now pending on management approval.
